On August 27, 2026, Licensing Program Analyst (LPA) Marilou Monico made an unannounced Complaint Inspection. LPA met with Site Director, Chinlan 'Jina' Wang, and explained the nature of today's visit. LPA toured the facility and interviewed staff.

During the investigation, LPA conducted observations and interviewed staff and parents. Based on interviews, on August 6, 2026, a child (C1) exited the facility unnoticed by staff and was later located outside the campus by a parent (P1). This incident was not reported to Licensing as required. Additionally, based on LPA’s observations and interviews, a licensing report documenting a Type A citation issued on August 6, 2026 pertaining to Teacher-Child Ratio, was not posted at the facility. Furthermore, parents were not provided copy of the licensing report documenting the Type A citation issued on that same date.

Based on LPA's observation and interviews, the preponderance of evidence standard has been met, therefore the above allegations are found to be SUBSTANTIATED.

Exit interview conducted and report was reviewed with Site Director, Jina Wang. Appeal rights were provided.

Per Title 22, Division 12, of the California Code of Regulations, the following Type A & Type B deficiencies are being cited: (see next pages).

LPA Monico informed Site Director, Jina Wang, that this report dated August 27, 2026 documents one Type A citation which shall be posted for 30 consecutive days as there is immediate risk to the health, safety, or personal rights of children in care. Also, LPA Monico informed the Site Director to provide a copy of this licensing report dated August 27, 2026 that documents any Type A citation to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification.

A copy of the Fact Sheet - Child Care Parent Notification Requirements and a copy of LIC 9224 Acknowledgement of Receipt of Licensing Reports was given to Site Director.

A Notice of Site Visit is provided and required to be posted for 30 days.

Site Director has posted the report related to the Type A deficiency issued on August 6, 2026 at the main entrance. Site Director also stated that she will submit a written plan by August 31, 2026, outlining how the facility will ensure ongoing compliance with posting requirements moving forward.
