LPA (Licensing Program Analyst) Anna Morales conducted an unannounced Annual/Required Inspection. LPA met with Licensee Soheila Salari, her husband, and one staff. Also, present was her adult son. Present in the home were nine(9) day care(three infants over 12 months and six preschool aged children). Children were being supervised by the Licensee and staff. Days and hours of operation are Monday - Friday from 7:30 AM -5:30PM. Adults over the age of 18 and residing in the home are the Licensee, her spouse and her son. All adults have Criminal Background Check Clearances. LPA reviewed a Children's Roster. Licensee will be participating with the Great Shake Out tomorrow, 10/17/24.

Licensee was reminded that all adults 18 and over living or working in the home, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.

This is a two story home and LPA observed a barricade in front of the stairs. The fire place inside the front living room is also barricaded. The off limit areas inside the home are: the entire second floor, the front living/dining room, kitchen and the garage. The off limits outside the home are: the left side of the back yard. Children will have access to the back living room(main day care area) part of the kitchen area, downstairs bathroom, and the two bedrooms downstairs.

LPA observed a enclosed playground (in the back yard) with age appropriate toys. LPA observed a fully charged 3A-40BC fire extinguisher, working smoke, and carbon monoxide detectors. LPA did not observe a body of water. The Licensee states that she does not have any weapons in the home. All detergents, cleaning compounds, poisons, medications, and other similar items are out of reach and inaccessible to children.
Staff stated has a first aid kit in the day care. Staff stated that nobody smokes, and she understands that smoking is prohibited in the day care.

LPA reviewed a random selection of children's file and observed that parent's rights forms, immunization records forms, consents for emergency medical treatment forms, and Identification forms are in each file. LPA observed that there Licensee was conducting and documenting 15 minute checks on infants under the age of 24 months old.

Licensee and Staff files were reviewed for the following records: Employee Rights (LIC9052), Statement Acknowledging Requirement to report Child Abuse (LIC9108), and Immunization Record showing immunity to measles (MMR), pertussis (Tdap), and influenza (or statement declining influenza).

LPA discussed the safe sleep regulations with Licensee and discussed the Child Care Licensing Safe Sleep webpage at:https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-andresources/safe-sleep as an additional resource. LPA also informed licensee of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

Licensee was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.

During the exit interview, the Licensee Sohelia Salari, confirmed that there are no Registered Sex Offenders living in the facility and LPA completed the RSO profile in FAS.

A notice of site visit was given and must remain posted for 30 days. No Deficiencies were cited during today's inspection.
