A second incident occurred on September 2, 2025, involving C2. During transition to the outdoor play area, S1 left C2 unattended in the classroom. Approximately five minutes after S1 exited, S2 entered the room and observed that C2 had been left alone.
Both incidents were self-reported by the facility to the Department. The video evidence and staff statements corroborate that the allegations occurred as reported. These incidents demonstrate lapses in supervision.
Based on the information indicated above, the preponderance of evidence standard has been met, therefore the above allegation is found to be SUBSTANTIATED California Code of Regulations, 101229(a)(1), are being attached on the LIC 9099D.
Also, LPA Andy Yang informed the licensee to provide a copy of this licensing report dated 9/08/2025 that documents any Type A citation to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification.
Due to Zero Tolerance Violation (ZTV), an immediate civil penalty of $500 shall be assessed on 9/08/2025.
A notice of site visit given and shall be posted for 30 days. Appeal Rights given.
