25 | Licensing Program Analyst (LPA), Shine Yu, conduct an unannounced visit for Annual/Random inspection and capacity increase request. LPA met with facility staff, Jianhui Hu. The licensee, Yu-Chuan Wang, was out for grocery purchasing and returned home 9:07 am. LPA explained the purpose of today's visit.
LPA observed the required posted materials, including the Facility License, Emergency Disaster Plan (LIC610) and Parents' Rights (PUB 393). Facility's operating days and hours are Monday to Friday 8:30 AM to 6:00 PM. LPA observed a current Children's roster. The last fire drill was conducted on 09/20/2025. Present are licensee, 1 staff and 4 infants.
LPA reviewed all 4 infants' files and observed that notification of parent's rights forms, immunization records forms, consents for emergency medical treatment forms, and Identification forms are in each file. LPA observed current 15 minutes sleep check log.
LPA also reviewed the staff files for the following records: Employee Rights (LIC9052), Statement Acknowledging Requirement to report Child Abuse (LIC9108), and Immunization Record showing immunity to measles (MMR), pertussis (Tdap), and influenza. Licensee's pediatric 1st Aid and CPR certificate, expires 08/22/2027 and her mandated reporter training certificate expired on 07/30/2026. The staff pediatric 1st Aid and CPR certificate, expires 06/03/2026 and her mandated reporter training certificate expired on 10/13/2027.
Licensee accompanied with LPA to tour the facility. The off limits areas inside the home are bedroom 2, 3 and garage. LPA observed the fire extinguishers (3A 40 BC) in the kitchen and outdoor play yard. The smoke and carbon monoxide detectors are in living room, bedroom and hall way. The knife, sharp objects and cleaning compounds are stored in the higher place where is inaccessible to the children.
