32 | For any injury or incident including challenging behavior, one or both children’s parents are provided a written report. Any contact with a parent is logged on a family contact log. Staff stated that they are provided with sufficient support and tools when dealing with children with challenging behaviors. Staff complete Behavior Observation Reports (BORs) to monitor what the child's triggers are and try connecting them so they can strategize and prevent future incidents.
If a child is showing repeated challenging behavior, they record it and talk to the parent. If a child is hurting others, they have a staff assigned to them and do activities like talk about safety, use emotional teaching solution cards, yoga and breathing, calming area, play soft music, read a book related to their behavior or give them a soothing item. If a child is being aggressive, they provide referrals to the mental health therapist and speak with the parents about the behavior, swap ideas and monitor their triggers. They are an inclusive classroom, and they try to work with the child to help them and try to be quick to stop any unsafe behavior. They keep the other children safe by creating a safe environment, separating the child, giving them more attention or moving them to a different table.
Based on records reviewed, LPA observed that BORs are completed for children showing challenging behavior. Staff record possible triggers, describe what happened and what action was taken by the staff. It also records the possible motivation of the child, where it occurred and who was involved. Staff also complete family contact logs to record any conversations with authorized representatives including relaying challenging behavior, techniques used and how they can help at home.
Based on parent interviews, parents stated that children are observed to be sufficiently supervised and staff communicate with them regarding incidents in a timely manner.
Based on interviews and evidence gathered at this time, it is concluded that although the allegation listed above may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur. The allegation is thus UNSUBSTANTIATED.
No deficiencies were cited as a result of today’s inspection. Exit interview conducted and report was reviewed with Director, Laura Fuentes. Appeal rights provided.
