The purpose of the visit was an unannounced pre-licensing inspection at the facility to ensure the health, safety and personal rights as required by Title 22 and Health and Safety Regulations governing California Child Care Centers will be met. The facility has an existing preschool program Facility #430702131 with a capacity of 144 preschool children which will be converted to a single license upon completion of the inspection process. Applicant is requesting to be licensed a capacity of 130 of which 112 are preschoolers (ages 2 - entry to 1st grade) in preschool rooms #1,2 & 4 and 18 infants (ages 2- 23 months) in infant room #3. Hours of operation will be Monday to Friday 6:30AM to 6:00PM. There are children present, and the teacher-to-child ratio complies with regulations during today’s inspection. Fire clearance was granted on 02/13/26. Facility is located in a church site - East Valley Pentecostal Church. LPAs toured and measured the on-limits areas and classrooms located at the facility. The facility has a total of 4 classrooms. Preschool Classroom measurements are: The Indoor total measurements contain a total of {3950.61} square feet, which will accommodate Applicant's request for 112 preschool children. Infant Classroom measurements are: The Indoor total measurements contain a total of {654.12} square feet, which will accommodate Applicant's request for 18 infants Preschool Outdoor Play measurements are: Outdoor total measurements contain a total of {10,912.70 square feet), which will accommodate Applicant’s request for 112 preschool children.

FACILITY EVALUATION REPORT California law requires a public report of each licensing visit/inspection. This report is a record for the facility and the licensing agency. This report is available for public review; therefore, care is taken not to disclose personal or confidential information. DEFICIENCIES A deficiency is an instance of noncompliance with licensing requirements, including applicable statutes, regulations, interim licensing standards, operating standards, and written directives. Applicants/ licensees must be notified in writing of all licensing deficiencies. Deficiencies are listed on the left side of this form, and the applicable licensing requirement upon which the deficiency is identified. There are two types of deficiencies:

Type A deficiencies are violations of licensing requirements that, if not corrected, have a direct and immediate risk to the health, safety, or personal rights of persons in care.

Type B deficiencies are violations of licensing requirements that, without correction, could become a risk to the health, safety, or personal rights of persons in care, a recordkeeping violation that could impact the care of said persons and/or protection of their resources, or a violation that could impact those services required to meet the needs of persons in care.

PLANS OF CORRECTION (POCs) The licensing agency is required to establish a reasonable length of time to correct a deficiency. In order to set the time, the licensing agency must take into consideration the seriousness of the violation, the number of persons in care involved, and the availability of equipment and personnel necessary to correct the violation. Applicants/licensees are requested to provide a specific plan for each violation on the right side of the form across from each deficiency. The more specific the plan, the less chance exists for any misunderstanding in setting time limits and reviewing corrections. The applicant/licensee who encounters problems beyond their control in completing the corrections within the specified time frame may request and may be granted an extension of the correction due date by the licensing agency.

CORRECTION NOTIFICATION The applicant/licensee is responsible for completing all corrections and promptly notifying the licensing agency of corrections. Applicants/licensees are advised to keep a dated copy of any correspondence sent to the licensing agency concerning corrections, or if corrections are telephoned to the licensing agency, the date, person contacted, and information given.

CIVIL PENALTIES The licensing agency is required by law to issue a Penalty Notice, when applicable, to all facilities holding a license issued by the licensing agency, or subject to licensure, except Certified Family Homes, Resource Families, and Foster Family Homes, or any governmental entity.

PENALTY NOTICE GIVEN The statement concerning civil penalties serves as a penalty notice on this Licensing Report and failure to correct cited licensing deficiencies will result in civil penalties. Applicants/ licensees are required to pay civil penalties when administrative appeals have been exhausted and in accordance with any payment arrangements made with the licensing agency.

APPEAL RIGHTS The applicant/licensee has a right without prejudice to discuss any disagreement in this report with the licensing agency concerning the proper application of licensing requirements. The applicant/ licensee may request a formal review by the licensing agency to amend or dismiss the notice of deficiency and/ or civil penalty. Requests for review shall be made in writing within 15 business days of receipt of a deficiency notification or civil penalty assessment. Licensing deficiencies may be appealed pursuant to the procedures in the LIC 9058 Applicant/Licensee Rights.

AGENCY REVIEW The licensing agency review of an appeal may be conducted based upon information provided in writing by the applicant/licensee. The applicant/licensee may request an office meeting to provide additional information. The applicant/licensee will be notified in writing of the results of the agency review within 60 business days of the date when all necessary information has been provided to the licensing agency.

EMAIL REQUIREMENT Adult Community Care Facilities, Residential Care Facilities for the Chronically Ill, and Residential Care Facilities for the Elderly are required to provide and maintain an active email address of record with the licensing agency.

Applicant was advised that indoor activity space for infants shall be physically separate from space used by children in the child care center per regulations. The preschool and infant programs will share the preschool playground on a rotating schedule. Applicant will submit a waiver for the shared use of the playground between the preschool and infant programs. LPAs also observed that the facility shares the preschool playground with children enrolled in East Valley Christian School which operates at the same church site and serves school age-children (elementary/middle school). Applicant will also submit a waiver request for the shared use of the preschool playground with the school-age children from East Valley Christian School.

All indoor and outdoor activity space utilized for the children were inspected today. LPA informed Applicant that staff are required to maintain direct visual supervision of the children at all times during indoor and outdoor activities. All classrooms were observed to be clean, safe, sanitary and in good repair. All outdoor and indoor passage ways and other areas of potential hazard were free of obstruction. A comfortable temperature for children shall be maintained at all times. Furniture and equipment were observed to be maintained in good condition, free of sharp, loose or pointed parts. All materials and surfaces accessible to children, including toys, shall be free of toxic substances. All play equipment and materials used by children were observed to be age-appropriate. LPAs observed no fireplace or open face heaters. LPAs observed fully charged fire extinguisher, smoke and carbon monoxide detectors in the facility. There are 110 chairs, 34 tables, 90 cubbies, 2 changing tables, 150 cots, 5 cribs, 4 feeding chairs, 13 toilets and 11 sinks available for the preschool and infant children. Staff have separate bathroom that can be utilized by sick children if needed. Sick children will be isolated in the director's office area of the facility. Applicant was advised that the isolation area shall be equipped with a crib, cot, mat or playpen for each ill infant. LPAs observed that items that may pose a danger to children were stored in cabinets inaccessible to children. LPAs observed containers with tight-fitting lids designated for the disposal of solid waste and diapers. LPAs observed trash bins without tight fitting lids which Applicant stated will be used solely for disposal of napkins used for hand washing. LPAs advised that Applicant maintains the first-aid supplies in a location accessible to staff but inaccessible to children.

Facility has one infant classroom. LPAs advised applicant that changing tables while in use, must be within arms reach to the sink. Applicant stated that parents will provide milk bottles and food for infants. LPAs advised Applicant that as per Title 22 regulations, bottles and dishes shall be rinsed for sanitizing at the end of each day. LPAs observed that the infant napping area does not have a separation from the infant activity area. LPAs advised that moveable partitions used for the crib area shall be at least four feet high; shall be constructed of sound-absorbing material; and shall be designed to minimize the risk of injury to infants.

LPAs observed the facility has a fully fenced outdoor playground area free of hazards including, holes, broken glass, dry grasses, and other debris that pose a fire hazard. The fence is at least four feet high. Any construction or equipment that could pose a hazard in the outdoor activity area shall be made inaccessible to children in care. All outdoor playground climbing equipment including a climbing dome are securely anchored to the ground. The areas around and under high climbing equipment were observed to be cushioned with poured rubber material and turf. There are age-appropriate outdoor toys and materials for the children. Shaded rest areas are provided through trees and a metal awning structure. Applicant understands that no child(ren) shall be left without visual supervision of a teacher at any time. Applicant stated that there are no weapons stored in the facility premises. LPAs observed no bodies of water. Facility does not provide transportation for the children.

The Applicant shall take measures to keep the center free of flies, other insects, and rodents. Snacks are prepared at the facility's kitchen, lunch are provided by parents. Applicant was advised that full-day programs shall ensure that each child has a lunch and children shall be free to drink as they wish. Drinking water is readily available for the children via water bottles from filtered water source. Children can bring their own water bottles outdoors. LPAs observed a kitchen/ food preparation area kept clean and free of litter and rubbish. Sink faucets accessible to children provide only cold running water.

Drinking water from a non-contaminating fixture or container shall be readily available both indoors and in the outdoor activity area. LPAs advised per Health and Safety Code 1597.16(a)(1) Lead Testing and PIN-21-21-CCP Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010 to test their drinking water for lead contamination between January 1, 2020 and January 1, 2023, and then every 5 years after the date of the first test.
Per Written Directive 100700(c)(1) Written Directives for Lead Testing (1) For a license issued on or after July 1, 2022, initial testing results shall be received and posted within 180 days of licensure.

LPAs discussed the safe sleep regulations with Applicant representative and discussed the Child Care Licensing Safe Sleep webpage at: https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep, as an additional resource. LPAs also informed Applicant of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at: https://www.cpsc.gov/, and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

LPAs reviewed with the Applicant LIC 311A, Records to Be Maintained At The Facility, for child’s records, personnel records, administrative records, and documents to be posted.
Applicant was informed of the MyChildCarePlan.org site, a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.

Community Care Licensing Division (CCLD) regularly sends information to licensed facilities, providers, and stakeholders by way of Provider Information Notices (PIN), Program Quarterly Update Newsletters and other important information communication platforms. To receive important licensed related information to licensed facilities, visit the CCLD Important Information website at https://www.cdss.ca.gov/inforesources/community-care-licensing/subscribe and select the Child Care option to receive email communication.

The following items are needed before a licensure will be considered.
1. Approval of waiver for shared outdoor space for preschool and infant yard use on rotation including shared use with the East Valley Christian School.
2. Approved fire clearance to reflect updated child capacity, room numbers and playground space eliminating playground B.
3. Updated LIC200A reflecting updated infant and preschool capacities.
4. Updated facility sketch showing updated room numbers of the preschool and infant rooms. Submit a separate sketch for the infant room.
5. Proof of available partition of crib area at least 4 feet high and constructed of sound absorbing material.
6. Approval of exception request for infant director.
7. Verification of age label stickers indicating the intended age group for the preschool playground climbing structure.

An exit interview was conducted and copy of appeal rights and this report was provided to the Applicant Representative: Dora Cortez. Final license determination will be made upon review and approval by the Licensing Program Manager.
