On 08/27/2025, Licensing Program Analyst (LPA) Kiran Jain arrived unannounced at the facility to deliver and discuss the findings of the Complaint allegations and investigation. Upon arrival, the LPA met with Director of Admissions (DOA), Michelle Ly and disclosed the purpose of the visit.

On 07/23/2025, the department received a complaint with one (1) allegation ‘Staff did not keep facility free of bed bugs’.

On 07/29/2025 and 08/20/2025, the department conducted initial investigations at the facility.

On 07/29/2025, during the facility visit, the LPA observed that the door to a first-floor room was closed, with a sign posted stating, “Room in treatment. Do not enter. Thank you.”

On 07/29/2025 and 08/20/2025, LPA interviewed five (5) staff members (S1-S5) and five (5) residents (R1–R5).

S1 stated that bed bugs had been an ongoing issue, and a resident previously experienced bites and later provided staff with a photo of a bed bug. S1 stated that medical care was offered, but was declined by the resident. At the time of inspection, no bed bugs were observed by staff, and the affected residents were relocated to another room. S1 further stated that chemical spraying and steaming were completed, and pest control services, including a professional inspection, did not find evidence of bed bugs. A K9 inspection was scheduled, and new residents later moved into the room without reporting any bites.

S2 stated that a resident observed a bed bug in a first-floor room, after which the room was blocked off, sprayed, and steamed. The resident had previously reported bites and was offered medical care, but was declined by the resident. S2 stated that an internal inspection at the time did not reveal bed bugs, and a professional exterminator was contacted only after the sighting of bed bugs by a resident who previously reported getting bites. Western Exterminator later inspected the room before new residents moved in and did not find evidence of bed bugs. S2 further stated that no complaints had been received from new residents.

S3 stated that Western Exterminator conducted a visual inspection on 07/31/2025 and confirmed no bed bugs were present. The room was sealed until after the inspection and later reopened for use, with no bug bite complaints reported by new residents. S3 further stated that when a resident previously reported bites, urgent care was offered, but was declined by the resident. At that time, no bed bugs were observed, and the source of the bites could not be confirmed.

S4 stated that a resident previously experienced bites, which were initially believed to be related to eczema. The resident declined an offer to seek urgent care. Staff confirmed that later, a bed bug was found by the same resident who previously experienced bites, after which the room was closed, the residents were relocated, and treatment was completed. S4 further stated that inspections did not confirm additional bed bug activity, and new residents who moved into the room two to three weeks later had not reported any issues.

S5 stated that there had been ongoing bed bug issues in a downstairs room, which had previously been treated by a pest control company, including a K9 inspection.

S5 stated that new bed frames, mattresses, and box springs were provided, and no further incidents were observed for some time, leading staff to believe the issue had been resolved. On 07/20/2025, a resident reported a bed bug incident in the downstairs bedroom. S5 stated that the affected residents were relocated, the room was isolated, and chemical treatments were applied, with a notice posted on the door indicating chemical use. S5 further reported that pest control services were contacted, a bed bug steamer was used, and all available measures were being taken to eradicate bed bugs.

R1 stated that R1 became aware of bed bugs in the first-floor room on July 20, 2025. R1 stated that the room had experienced bed bug issues previously and that staff had indicated mattresses and bed frames were replaced and a K9 inspection was conducted, though R1 did not personally observe this. R1 stated that recent spraying had been performed, but staff could not confirm the room was safe, leading R1 to move upstairs while the roommate slept on the couch. R1 stated not experiencing bites personally, but stated that their roommate had reported bites to staff for approximately three weeks. R1 stated that these concerns were initially dismissed as mosquito bites, with no medical evaluation provided. R1 further stated that the roommate eventually took a photo of a bed bug on the roommate’s face, which prompted staff to strip bedding, wash clothing, and use a bug treatment machine. R1 stated that spraying was performed by facility staff rather than a professional pest control company.

R2 stated that R2 moved into the downstairs bedroom and was aware that R2’s room had a prior bed bug infestation that was treated. R2 stated that during R2’s stay, had neither seen nor been bitten by bed bugs.

R3 stated that R3 was residing in the downstairs bedroom and R3 was not aware of any bed bug infestation. R3 further stated that R3 had not experienced any bites and had not observed any bed bugs in R3’s room.

R4 stated that they lived in an upstairs room but spent significant time in the downstairs room. The occupant of the downstairs room discovered bed bugs on the bed and shared a photo confirming the infestation. R4 stated that staff stripped the bedding and washed it, but initial actions were limited, with staff later cleaning the room and posting a chemical-use sign. R4 stated that R4 had not seen an exterminator, and residents did not receive updates before new residents were moved into the room. R4 added that staff became upset when residents discussed the bed bug situation.

R5 stated that a bed bug was found in a downstairs bedroom, after which the occupants slept in the living room due to bites, and staff removed the mattress.

R5 was unsure whether the room was treated or inspected, but recalled being told that a dog had been brought in to detect bed bugs, though R5 was uncertain when this occurred. R5 added that R5 would not live in the downstairs room.

On 08/01/2025 and 08/20/2025, LPA received and reviewed the facility’s Pest Control service inspection and treatment reports from 04/29/2025 to 08/20/2025. Between April and August 2025, the facility obtained multiple pest control services from Western Exterminator (Rentokil). On 04/30/2025, a treatment was performed after the facility reported bed bug activity, noting high bed bug activity levels. During 05/14/2025 service, Rentokil found no bed bugs but applied preventive treatments. On 06/27/2025 and 08/20/2025, a K9 inspection was conducted with no evidence of live bed bug activity in the home.

On 08/01/2025, LPA received and reviewed the facility’s bed bug Remediation Plan, which stated they will incorporate monthly bed bug inspections along with their monthly pest control services with Terminix or Western. Staff will also be purchasing bed bug traps for ongoing prevention. They are also setting up bi-yearly K9 inspections through Western Exterminators.

On 08/20/2025, the LPA reviewed the facility’s file for past Incident Reports and found an Incident Report dated 07/20/2025, in which a client informed staff that the client found a bed bug in the client’s room.

Based on observations, interviews conducted, and records reviewed, 5 of 5 staff and 4 of 5 residents acknowledged that bed bugs had been an issue. 5 of 5 staff and 3 of 5 residents confirmed that a resident experienced bites starting in early July 2025. The resident's initial bite concerns were dismissed as mosquito bites, and the resident had to provide proof of bed bugs present in their room. Pest control service records documented a high level of bed bug activity in April 2025, and there was a confirmed additional bed bug activity in July 2025 when a resident found bed bugs in their room. Despite regular treatments and inspections, bed bugs continued to be present in the facility. The preponderance of evidence standard has been met, therefore the above allegation is found to be SUBSTANTIATED.

A deficiency is being cited in accordance with the California Code of Regulations, Title 22, see LIC 9099D.

An exit interview was conducted, and the Plan of Correction was reviewed and developed with the Director of Admissions. A copy of this report and appeal rights were discussed and left with the Director of Admissions, Michelle Ly, whose signature on this form confirms receipt of these documents.

The Director of Admissions stated they will submit a written plan detailing the steps and procedures that will be implemented to address and remediate the bed bug infestation moving forward. The plan shall be submitted to the CCL by the POC due date of 09/26/2025.
