Licensing Program Analyst (LPA) Maria (Mita) Partoza conducted an unannounced required 1 year inspection visit, and met with LIcensee Iluminada Navarro and stated the purpose of the visit. Administrator Remil Navarro was not present at the time of LPAs arrival. Administrator came at 3:30 p.m.

Licensee contacted the administrator because licensee cannot access the resident files and staff file, that is required to be inspected during annual inspection. The facility is licensed to serve 18 through 59 who has mental disability. The facility can accommodate 6 ambulatory.

LPA observed that 2 licensee Remigio and Iluminda Navarro were present and 2 residents. LPA toured the facility inside and outside including the living room, kitchen, dining room, 2 restrooms, 4 resident bedrooms, backyard, garage and side emergency exits. LPA observed hallways and walkways to be free from obstructions. Residents room has ample storage for their personal belonging. LPA inspected food storage and observed 2 days of perishable and 7 days of nonperishable food supplies. LPA observed the medication, knives were locked and not easily accessible to residents. The room temperature is at 78 degrees F, and hot water temperature was measured at 112.4 degree F with a digital thermometer. The refrigerator temperature is at 32 degree F and freezer is at 0 degree F

Fire extinguisher was serviced in June 10, 2025. The facility is equipped with a fire alarm wall pull system and smoke detectors.

FACILITY EVALUATION REPORT California law requires a public report of each licensing visit/inspection. This report is a record for the facility and the licensing agency. This report is available for public review; therefore, care is taken not to disclose personal or confidential information. Inquiries concerning the location, maintenance, and contents of these reports may be directed to the Licensing Program Analyst or Regional Office whose address and telephone number are listed on the front of this form.

DEFICIENCIES A deficiency is an instance of noncompliance with licensing requirements, including applicable statutes, regulations, interim licensing standards, operating standards, and written directives. Applicants/ licensees must be notified in writing of all licensing deficiencies. Deficiencies are listed on the left side of this form, and the applicable licensing requirement upon which the deficiency is identified. There are two types of deficiencies:
  • Type A deficiencies are violations of licensing requirements that, if not corrected, have a direct and immediate risk to the health, safety, or personal rights of persons in care.
  • Type B deficiencies are violations of licensing requirements that, without correction, could become a risk to the health, safety, or personal rights of persons in care, a recordkeeping violation that could impact the care of said persons and/or protection of their resources, or a violation that could impact those services required to meet the needs of persons in care.

PLANS OF CORRECTION (POCs) The licensing agency is required to establish a reasonable length of time to correct a deficiency. In order to set the time, the licensing agency must take into consideration the seriousness of the violation, the number of persons in care involved, and the availability of equipment and personnel necessary to correct the violation. Applicants/licensees are requested to provide a specific plan for each violation on the right side of the form across from each deficiency. The more specific the plan, the less chance exists for any misunderstanding in setting time limits and reviewing corrections. The applicant/licensee who encounters problems beyond their control in completing the corrections within the specified time frame may request and may be granted an extension of the correction due date by the licensing agency.

CORRECTION NOTIFICATION The applicant/licensee is responsible for completing all corrections and promptly notifying the licensing agency of corrections. Applicants/licensees are advised to keep a dated copy of any correspondence sent to the licensing agency concerning corrections, or if corrections are telephoned to the licensing agency, the date, person contacted, and information given.

CIVIL PENALTIES The licensing agency is required by law to issue a Penalty Notice, when applicable, to all facilities holding a license issued by the licensing agency, or subject to licensure, except Certified Family Homes, Resource Families, and Foster Family Homes, or any governmental entity.

PENALTY NOTICE GIVEN The statement concerning civil penalties serves as a penalty notice on this Licensing Report and failure to correct cited licensing deficiencies will result in civil penalties. Applicants/ licensees are required to pay civil penalties when administrative appeals have been exhausted and in accordance with any payment arrangements made with the licensing agency.

APPEAL RIGHTS The applicant/licensee has a right without prejudice to discuss any disagreement in this report with the licensing agency concerning the proper application of licensing requirements. The applicant/ licensee may request a formal review by the licensing agency to amend or dismiss the notice of deficiency and/ or civil penalty. Requests for review shall be made in writing within 15 business days of receipt of a deficiency notification or civil penalty assessment. Licensing deficiencies may be appealed pursuant to the procedures in the LIC 9058 Applicant/Licensee Rights.

AGENCY REVIEW The licensing agency review of an appeal may be conducted based upon information provided in writing by the applicant/licensee. The applicant/licensee may request an office meeting to provide additional information. The applicant/licensee will be notified in writing of the results of the agency review within 60 business days of the date when all necessary information has been provided to the licensing agency.

EMAIL REQUIREMENT Adult Community Care Facilities, Residential Care Facilities for the Chronically Ill, and Residential Care Facilities for the Elderly are required to provide and maintain an active email address of record with the licensing agency.

During inspection, the Licensee stated that he/she cannot access the file room. Resident and staff record were not available for inspection. Administrator was called by the licensee, and ADM was able to access the record for LPAs inspection when he/she arrived at the facility.

LPA observed the door that access the garage from the inside is not locked. Inside the garage are chemicals and laundry detergents that were easily accessible to residents in care. Licensee locked the garage door when LPA stated that the door has to be locked if there are chemicals and if the facility is storing chemicals in the garage.

LPA gave the administrator technical advisory on the last annual inspection 4/23/2025, regarding the upkeep and maintenance of the surfaces in the resident's room, dining room, kitchen and bathroom that are used by the resident. LPA observed that the bathroom used by the residents have residue that have not been wiped or sanitized. LPA observed that 1 out 2 residents' bedroom floor has visible debris, wrappers under the bed. 1 out of 2 resident bedroom was observed to be free from debris and sanitary. 4 out of 4 bedroom were inspected. 2 out of 4 were vacant, 1 staff room was locked and 1 file room.

LPA interviewed 2 residents, 1 out of 2 resident stated that they were not happy with the food that is being served. 1 out of 2 did not say give any comment on food services.

Surfaces such as floors, chairs, toilets, sinks, counters and tabletops shall be cleaned and disinfected on a regular basis to ensure they are safe and sanitary.  These surfaces shall also be disinfected when these surfaces are contaminated and visibly soiled with blood or body fluids or other potentially infectious material. 

LPA observed the following, scissors in the sink and in one of the utensil drawers, the scissor has a sharp point. LPA observed empty cans of energy drink on a table next to the refrigerator, a sack of rice under the table and a container of cooking oil.

LPA reviewed the 2 resident file 1 out of 2 was missing appraisal needs and services plan, 2 out of 2 resident have complete physician's report, centrally stored medication and destruction record (CSMDR). 1 out of 2 resident has personal and incidental managed by the facility. The facility conducted the fire drill on 04/01/2026.

Deficiencies are cited during today's visit based on California Code of Regulations (CCR) Title 22. See LIC 9099D

85068.3 Modifications to Needs and Services Plan (a) The written Needs and Services Plan specified in Section 85068.2 shall be updated as frequently as necessary to ensure its accuracy, and to document significant occurrences that result in changes in the client's physical, mental and/or social functioning.
