32 | On January 9, 2025, LPA Manuel Monter interviewed Staff S1. S1 stated the day of the elopement, he/she was in the home with 2 residents. S1 stated resident R2 was leaving and requesting to take some P&I money. S1 stated during that transaction, R1 had left the facility. S1 stated after he/she had finished giving the P&I, S1 stated he/she noticed R1 was no longer in the home. S1 stated this occurred a few minutes before 2:00pm. S1 stated he/she searched the home for R1, and when he/she could not find R1, he/she contacted the ADM. S1 stated he/she then went to search outside the home and as he/she was walking up the street, S1 stated he/she saw the police, neighbors and R1, on Tennant Ave, between Southlite Ct and S Breeze Ct. S1 stated, the police brought R1 back to the facility. S1 stated R1 did not have any injuries.
Based on a google maps search, the distance between the facility, and where staff S1 stated R1 was found is 0.1 miles away from the facility.
On January 9, 2025, LPA observed the front of the home no longer had the garage door. LPA observed the front of the home, where the garage door was located, now has a door and two windows. (Photographs were taken.) LPA asked ADM if she notified Community Care Licensing, prior to constructions. ADM stated no. ADM sent LPA email a copy of the building permits.
Based on a review of R1’s Physicians Report, dated September 15, 2024, R1 cannot leave the facility unassisted. A review of R1’s Appraisal/Needs & Services Plan, dated February 23, 2024, one of R1’s listed is, “attempts of eloping.”
Based on a review of R1’s Individual Program Plan (IPP), dated November 17, 2023, R1’s IPP states (Page 4), “R1 has a history of eloping from school or home.”
An immediate civil penalty of $500.00 is being assessed against the facility today for violation the absence of supervision, wherein R1 eloped from the facility on December 27, 2024.
