25 | Licensing Program Analyst (LPA) Maria (Mita) Partoza conducted an unannounced required 1 year inspection visit and met with Designated Program Director (DPD) Joshua Fane and stated the purpose of the visit. The facility is licensed as a Short-Term Crisis Residential Program for six (6) ambulatory.
At 9:45 a.m. LPA toured the facility with DPD, including but not limited to the kitchen, dining, residents bedrooms, bathrooms, group room, office, garage, the exterior perimeter, driveway, detached garage, fenced field at the end of the property.
Facility's temperature is at 74 degree Fahrenheit, the kitchen, living room dining room, group activity room and office were observed to be sanitary and organized. Knives and sharps are locked and not accessible to clients. LPA observed 2 days of perishable food and 7 days of non-perishable food. The water temperature measured at 112.8 degree Fahrenheit. LPA inspected 4 client bedroom and observed bedrooms to be sanitary, organized and has sufficient storage. Bathrooms are equipped with grab bars and non-skid mats. Cleaning supplies, laundry detergents are locked and not accessible to client. The facility is equipped with a fire, smoke and carbon monoxide alert system that is in good working condition, night lights are in good working condition and facility has first kits available to staff. Medication room is not accessible to clients.
The pool area is fenced in and is not easily accessible to clients, two clinical offices building used by licensed therapists and case managers. The driveway is free from obstruction. The backyard and walkways are maintained and free from debris and obstructions. The detached garage is not used as a living space but a storage used by the landlord. The facility has a septic system that is currently being maintained.
LPA reviewed 3 resident records such as but not limited to the centrally stored medication and destruction record (CSMDR), admission agreement, needs and services plan, health screening and observed records to be updated and complete. LPA reviewed 3 staff records including but not limited to required training, first aid/CPR training, health screening and background clearance. All staff have criminal record clearance/fingerprints.
No deficiencies were cited during today's visit based on California Code of Regulations (CCR) Title 22. An exit interview was conducted with Designated Program Director Joshua Fane and a copy of the report was provided. |
