32 | LPA observed 2 pet dogs during the inspection. The home has a fully charged 3A40BC fire extinguisher, working smoke detector, working carbon monoxide detector and working telephone. The fireplace is blocked by bookshelves to prevent access by children. There are no firearms in the home as per the licensee. The licensee owns the home; he does not carry childcare liability insurance or a bond and maintain the signed form LIC 282 AFFIDAVIT REGARDING LIABILITY INSURANCE.
The licensee completed the Health and Safety training completed on 10/23/2023, and CPR (Pediatric) and First Aid were renewed on 2/8/2025. The licensee renewed his Mandated Reporter Training on 10/22/2025. The licensee has records of Measles and Pertussis immunization, Influenza declination statement and TB clearance. LPA reminded the licensee that only the Influenza vaccination can be decline with a written declination.
The licensee was reminded that all adults 18 and over living in the home, persons who provide care and supervision to children, and staff who have contact with children, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.
Facility rosters of children were reviewed, and a copy was obtained. Children’s files were reviewed, which included but not limited to LIC 9150, LIC9227, records of receipt for Parents' Rights Notice, Identification and Emergency Information, Consent for Emergency Medical Treatment form, sleep logs and Immunization. The licensee is in ratio today. The licensee is currently not providing transportation to children in care. The licensee is currently providing overnight care. LPA reminded the licensee of the Overnight Care - Section 102426.
Incidental Medical Services (IMS) policy was discussed. For IMS information see Evaluator Manual - Regulation Interpretations and Procedures for Family Child Care Homes Section 102417. When any IMS is provided, a Plan for Providing IMS must be submitted to the Department.
