32 | 1. On 4/7/2026 at 11:44 am, during an indoor activity at one of the preschool classroom areas, Staff 5 (S5) was supervising 22 preschool children.
In addition, on 4/1/2026 at 10:47 am in the toddler room, during a circle time, Staff 6 and 7 (S6 and S7) were supervising 14 toddlers, and at 10:52 am, S6 and S7, were supervising 13 toddlers. As a result, the licensee was not in compliance with the section - Care for Toddlers within a Licensed Preschool Center WD 101804(b)(2) - a ratio of six children to each teacher shall be maintained for all children in attendance in the toddler component.
The preponderance of evidence standard has been met; therefore, the above allegation is to be SUBSTANTIATED. Section 101216.3(a) is cited on the LIC 9099 D page.
LPA Estoesta informed the Director that this report dated 7/1/2026, included a Type A deficiency which shall be posted as there is an immediate risk to the health, safety, or personal rights of children in care, for 30 days & provide copies of this report to parent/guardians of;
(1) children in care at the facility
(2) children newly enrolled at the facility during the next 12 months
The LIC 9224 - Acknowledgement of Receipt of Licensing Reports which is to be placed in each child's file.
An immediate civil penalty of $500 is being assessed and $100 will accrue until the deficiency is corrected.
A notice of site visit was given and must remain posted for 30 days.
Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
Exit interview conducted and report was reviewed with the Director, Muthulakshmi Dhivakaran.
