32 | Files and Record Keeping: LPA reviewed with Applicant the LIC 311 A, Records to Be Maintained At The Facility, for child’s records, personnel records, administrative records, and documents to be posted. Applicant understands the requirement to main current a current facility roster and personnel report. Applicant and Director have a fingerprint clearance and/or exemption and have been associated to the facility. Applicant and Director’s CPR/First-Aid Certificates and Mandated Reporter (Child Care Providers) Training Certificates are current. The center will use a paper sign-in/out system. Applicant understands that a fire/disaster drill must be conducted at least once every 6 months. Per Applicant, there are no firearms on the premises. All required documents, including the snack menu and waiver, are posted in a publicly accessible area by the entrance. LPA discussed 100% supervision is always required in all areas that children have access to including the bathroom. Requirements relating to Personal Rights, inspection authority, reporting requirements, maintaining buildings and grounds were reviewed. Staff Qualifications, Staff to Children’s ratios and Capacity were explained to ensure compliance. LPA discussed with Applicant any changes that may occur regarding the director or an employee acting in the director's absence must be reported to department within 10 working days.
Record Keeping and Reporting Requirements: Applicant was reminded that EMSA approved Pediatric CPR & First Aid training must be completed every two (2) years. Mandated Reporter Training ("Child Care Providers") is required for all staff and is to be renewed every 2 years by visiting https://mandatedreporterca.com/. Facility roster and personnel report must be properly maintained, and fire/disaster drills must be conducted at least every six (6) months and documented. Effective August 1, 2003 California Law requires Child Care Licensees to report unusual incidents or injuries to children in care to child’s parents and to the Department of Social Services using the Unusual Incident/Injury Form (LIC 624). Incidents must be reported within 24 hours to the regional office by phone and the written report, LIC 624, within seven (7) business days. LPA informed Director that all forms can be downloaded at www.ccld.ca.gov.
Criminal Record Clearance: Applicant was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.
