25 | On 03/16/2026, Licensing Program Analyst (LPA), Syeda Bahar, conducted an unannounced Case Management investigation regarding an incident that was reported to the department on 02/27/2026. Upon arrival LPA met with the, Program Coordinator, Vanessa Garcia and Site Supervisor, Gloria Leano Veneges and explained the purpose of the visit today. LPA toured the indoor and outdoor areas of the facility and a census was taken.
Per the incident report, on 02/26/2026, a preschool child (C1) was unsupervised for approximately 30 to 60 seconds in the bathroom. A staff (S1) from the other classroom entered through the outside door of the bathroom and found the child. No injuries were reported. During today’s investigation, LPA reviewed records, conducted staff interviews, obtained copies of personnel reports and facility roster and other supporting documents.
Based on interviews, Staff reported that the child had gone into the bathroom alone. The door closed behind the child, and staff did not maintain visual supervision for a short period of time (30-60 seconds). The child was located by staff (S1) from classroom 1 immediately. No hazards, or unsafe conditions were reported in the bathroom at the time of the incident. The child returned to class with no observed injuries or distress.
Based on interviews and record review, it was determined that a child (C1) in care was left without direct visual supervision for approximately 30 to 60 seconds.
As a result, one (1) Type B deficiency is cited on attached page 809-D. Appeal Rights were provided.
Exit interview was conducted, and the report was reviewed with the Site Supervisor, Gloria Leano Veneges and Program Coordinator, Vanessa Garcia.
A Notice of Site Visit was given and must remain posted for 30 days.
