25 | Licensing Program Analyst (LPA) Marilou Monico conducted a Case Management inspection in response to an incident that was self-reported by the facility on October 10, 2024 and to obtain signature for an amended report. LPA toured the facility both indoor and outdoor and interviewed staff. LPA met with Preschool Site Director, Tina Burcher, and School Age Program Director, Drew Saldivar. Tina left during the inspection.
LPA learned that on October 9, 2024, a daycare child, C1, exited the playground unnoticed by staff and had gone out the gate. Staff were searching for the child until they heard from the intercom of the school that a toddler was by the picnic table on the school campus. The picnic table is located outside the gate/fence area of the playground. C1 was retrieved by staff and was brought to the facility.
As a result of this inspection, Type A deficiency was cited on the following page.
Assembly Bill (AB) 633 was provided and discussed with the School Age Program Director, Drew Saldivar. LPA informed Drew to provide a copy of this licensing report dated October 16, 2024 that documents a Type A citation to parents/guardians of all children currently enrolled no later than the next business day or the next day the children are in care, and to parents/guardians of any newly enrolled children for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC9224) must be placed in the child's file for verification.
Exit interview conducted and report was reviewed with School Age Program Director, Drew Saldivar.
A Notice of Site Visit was issued and must remain posted for 30 days.
