Licensing Program Analysts (LPAs), Marilou Monico and Jovani Dillon, conducted an unannounced follow-up complaint inspection. LPAs met with Teacher/Director, Suzanna Lopez, Comprehensive Services Manager, Corey Vestal, and Site Supervisor, Arienya "Bryce" Chisterson.

Based on interviews and available evidence, a child (C-1) was left unattended outside Baskin 2 Room on 03/01/23 during transition from lunch to nap. The incident was self-reported by the facility to Licensing Office on 03/02/23. LPA Monico learned from interviews that a child's (C-2's) hair was pulled by staff (S-1). The above allegations are SUBSTANTIATED, meaning the allegations are valid because the preponderance of the evidence standard has been met.

The staff member (S-1) involved was given a written warning. S-1 attended training on active supervision on 04/20/23. Copy of written warning, training agenda and proof of attendance were submitted during the inspection.

Site Supervisor stated that she will submit a written plan by 06/16/23 to Licensing detailing the steps the facility are planning to take to ensure that children's personal rights are not violated.

Upon receipt, licensee shall post and provide copies of this licensing report to parents/guardians of children in care at the facility and to parents/guardians of children newly enrolled at the facility during the next 12 months.
Failure to correct the cited deficiency(ies), on or before the Plan of Correction (POC) due date, may result in a civil penalty assessment.
