25 | Licensing Program Manager (LPM), Joel Segura, and Licensing Program Analyst (LPA), Marilou Monico, met with Education Manager, Lorena Gonzalez, and Children and Family Development Programs Director, Anne Adamson, at the San Jose Regional Office today for a scheduled Informal Meeting. The purpose of today's meeting is to discuss the following deficiencies:
This informal conference is a direct result of recent Type A citations:
06/15/23 - Section 101229(a)(1)- Care and Supervision - On 03/01/23, a child (C-1) was left unattended outside Baskin 2 Room. Civil penalty of $500.00 was assessed.
06/15/23 - Section 101223(a)(2) - A staff member (S-1) pulled child's (C-2's) hair.
06/15/23 - Section 101223(a)(2) - A staff member (S-1) pinched Child #1's arm.
The facility implemented the following plans to bring the facility in compliance:
1) The staff member involved was terminated.
2) All staff including newly hired completed active supervision training.
3) Unannounced classroom visits to the facility and staff check ins by Site Supervisor and/or Education Manager for close monitoring and additional training support.
4) A licensing training refresher will be provided to all teaching staff.
Education Manager agreed to provide ongoing staff training on care and supervision, head counting, positioning of staff, and children's personal rights. Training agenda and proof of attendance shall be maintained at facility for Community Care Licensing review.
LPM explained to Education Manager that if there are continued deficiencies cited for the issues noted on this report, the Facility may be referred to legal department for possible administrative action, which could include revocation of the Facility license. The Facility will be monitored for the next 12 months at the department's discretion to ensure that the Facility is in compliance with the Department regulations.
Assembly Bill 633 (Child Care Parent Notification Requirements) and Acknowledgement of Receipt of Licensing Report (LIC9224) was also explained and provided to Education Manager during today’s meeting. |
