Licensing Program Analyst (LPA), Cortney Nelson, met with Licensee, Sandra Luera, to conduct pre-licensing inspection for change of location and capacity increase. Upon arrival, LPA was admitted into the home by Sandra and toured inside and outside during todays inspection.

The operating hours for the Family Child Care Home (FCCH) will be Monday - Friday from 6:00AM-6:00PM and will be open on Saturday 6:00AM-6:00PM as needed. The FCCH will care for children ages zero through thirteen. The FCCH currently works with Migrant Head Start and Go Kids. Sandra states that adults over the age of 18 residing in the home are herself and her daughter (Kassandra). There are three minor children living in the home. LPA advised that children living in the home will be included in the home ratio and capacity until they turn 10 years of age. Sandra has submitted proof of control of property via a rental agreement for the home and she states that she currently has liability insurance for the day care.

The Licensee was reminded that all adults 18 and over living in the home, persons who provide care and supervision to children, and staff who have contact with children, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

LPA toured inside the home and observed sufficient materials and play equipment for day care children. There are a variety of toys for the children to play with, including cars/vehicles, blocks, cooking/kitchen, and other manipulative materials. The wall heaters located inside of the home are properly barricaded. Off limits inside the home include: three bedrooms, kitchen, and the laundry room. LPA observed there are baby gates in the home to prevent children from accessing off-limits areas and the bedroom doors will remain locked during day care hours. LPA advised if there are changes to the on/off limits of the home to submit an updated facility sketch (LIC999A) to the San Jose Regional Office.

There is a working telephone at the FCCH. Sandra states that she currently has five nap mats and is planning to purchase more. There is a baby changing pad located in the restroom for changing diapers and a waste bin with a tight fitting lid for disposal. Sandra states that she does not plan to assist with potty training. There are no cribs/ pack n plays at the FCCH and Sandra states that Migrant Head Start will provide the equipment if they place an infant in her care.

Sandra states that she will provide medication and Incidental Medical Services to children as needed. The FCCH will work with Community Bridges Food Program and provide children in care breakfast, lunch, and AM/PM snack. Water will be provided to the children via plastic disposable water bottles. LPA observed kitchen knives are properly stored inaccessible to children. There is a functioning carbon monoxide detector, smoke detector, and fully charged 3A40BC fire extinguisher (last serviced: 5/2023).

Incidental Medical Services (IMS) policy was discussed. For IMS information , see PIN 22-02-CCP. When any IMS is provided, a Plan for Providing IMS must be submitted to the Department. A fire clearance for the home was approved on 11/29/2023 by the Watsonville Fire Department. The clearance indicates that the outside gates must remain closed to all other areas and movement from outside to the bathroom shall be supervised. The garage shall additionally be closed at all times while children are present. LPA reviewed conditions of the fire clearance with Sandra during today's inspection.

The outside area of the home was inspected and observed to be fenced in. Sandra states that the Fire Marshal advised that gates to exit the outside space remain unlocked in case of emergency. LPA observed that the gates have a self-latch attached, that is inaccessible to children. Off-limits outside of the home include: separate garage and the yard area beyond the gate. Sandra states that outdoor play equipment will be provided by Migrant Head Start once she receives her license and LPA requested that Sandra submit photos of the equipment once received.

LPA discussed isolation of sick children with Sandra and she states that if a child were to start feeling sick at her home that she would isolate the child in the living room or hallway with a nap mat, until the parent/guardian arrives to pick-up. LPA discussed isolation strategies, such as using a baby gate, and utilizing the restroom if the child's symptoms need close restroom access. LPA observed the Applicant has a first-aid kit, and thermometer, available in her home.

LPA reviewed supervision of children and requirements for additional staff/ adults living in the home. Sandra states that she transports children and LPA reviewed the vehicle (Dodge Caravan) and observed it is in good working condition. Sandra has car insurance through Kemper Auto Infinity and her CA drivers license is valid. The vehicle is equipped with one car seat and Sandra states that she transports school-age children. LPA requested that Sandra submit proof of current vehicle registration and insurance.

LPA reviewed large family child care home capacity requirements with Sandra and provided a copy of 102416.5 Staffing Ratio and Capacity from California Code of Regulations. LPA advised that there should never be more than four infants, children under two years of age, present at the FCCH. If no assistant is present, then the FCCH shall comply with the capacity requirements for a small FCCH. LPA advised that an assistant shall be at least 14 years old and, if under the age of 18, shall never be left alone with day care children.

LPA discussed the safe sleep regulations with the Licensee and provided the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource.

Sandra was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.

Exit interview conducted and report was reviewed with the Licensee, Sandra Luera.

LPA advised Sandra that a large FCCH license will be issued pending manager review/approval and submission of the following corrections:
-Proof of current vehicle registration and insurance for transporting children
-After license received, submit photos of outside play space with toys and equipment from MHS
-Proof of purchase for additional nap mats/cots
