Licensing Program Analysts (LPAs) Dung Mac and Samantha Yip, conducted an unannounced pre-licensing inspection to the Facility today. LPAs met with Toby Salciccia, the Applicant, Mary Ellen Spanu, Applicant Representative, Alejandrina Perez, Site Director, and explained the nature of today's inspection. The Applicant has applied for a preschool (#444417012) and infant (#444417013) on the same premises.

The Preschool program will operate Monday through Friday from 07:00AM to 06:00PM, serving Preschool children ages 2 years to Kindergarten. The Preschool program will be utilizing Rooms A, B, C, and Rec Rooms. The Infant program will be using Rooms D and E.

LPAs toured and measured the indoor and outdoor areas of the Facility during today's inspection.

INDOOR MEASUREMENTS ARE AS FOLLOWS:

Room A = (19.333 x 4.000) + (25.333 x 19.333) minus
(encumbered) (1.583 x 7.333 + 10.000 x 1.250)
= 567.094 - 24.108 = 542.986
Room B = (4.417 x 19.167) + (22.000 x 19.417) = 84.660 + 427.174 = 511.834
Room C = (19.417 x 4.000) + (23.750 x 19.250) = 77.668 + 457.187 = 534.855
Rec. Room = (22.083 x 21.917) minus (encumbered) (12.500 + 6.250)
= 483.993 - 18.750 = 465.243

TOTAL INDOOR SPACE = 542.986 + 511.834 + 534.855 + 465.243
= 2,054.918 divided by 35 = 58 children

LPAs observed functioning smoke/carbon monoxide detectors at the facility. Cleaning supplies are stored in an upper cabinet in each classroom. First aid supplies are stored in the classroom inaccessible to the children. Medications will be stored on the high shelves out of children's reach if administrating at the facility. Solid waste containers with tight-fitting lids are located in each classroom. All classrooms have appropriate lighting.

There are 6 toilets (90) and 5 sinks (75) available for the children. Only cold water is available in the children's sinks. Staff have a separate bathroom which is located between Rooms C and D. The sick child will be isolated in the Staff Room and isolation equipment is provided. The sick child will utilize the staff bathroom.

There are 97 cubbies, 13 tables, 65 chairs, and 61 mats. There are adequate equipment, supplies and toys for children. Children will bring their own lunch and snacks. Drinking water will be readily for children indoors via individual bottle and tap water. Facility will provide paper cups and plastic utensils as needed. LPAs observed kitchen is equipped with a sink, stove, microwave, and a refrigerator. The kitchen sink is currently out of order and in the process of repair. No transportation will be provided by the facility.

The playground is currently under renovation. Applicant stated that the estimate date of completion will be 04/15/2022. Applicant stated he will notify Licensing once renovation is completed. The follow-up inspection will be conducted once the playground is fully setup to determine outdoor square footage. A plan is in place to allow children to have outdoor space until outdoor square footage is determined. No bodies of water were observed.

Applicant was reminded that all adults 18 and over responsible for administration or direct supervision of staff, persons who provides care and supervision to children, and staff who have contact with children, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

LPAs reviewed with the Applicant and Facility Representative the LIC 311A, Records To Be Maintained At The Facility, for child’s records, personnel records, administrative records, and documents to be posted.

Exit interview conducted and report was reviewed with the Applicant, Toby Salccicia. Applicant submitted to the Licensing office an application for a provisional license for 90 days. Updated application documents, renovation of the outdoor activity space, and completion of the fire alarms must be completed within 90 days.
