Licensing Program Analysts (LPAs) Mel Matos & Cynthia Tamayo, conducted an announced prelicensing inspection at the facility today. Purpose of today's inspection: address emergency change of location request as a result of flood damage sustained at the licensed site located at 2800 Porter St, Soquel, CA 95073 (#444414415). LPAs met with Sara Balla, Applicant Representative, and explained the nature of today's inspection to her. Current site is located on the premises of the Santa Cruz Bible Church in Building CC (Rooms CC11-CC17).

The Applicant Representative is applying for a license to serve 40 preschool children ages 2 to 5 years of age. The Facility will operate Monday through Friday from 7:00AM - 4:00PM. Sara states that the Facility will have exclusive use of Rooms CC11-CC17, bathrooms, and playground area during operating hours.

LPAs toured and measured the indoor and outdoor areas of the Facility during today's inspection.

Indoor Activity Space:

Room CC11 = 23.250 x 19.000 = 441.750 minus (encumbered: 3.370 x 3.917 = 13.200) = 428.550

Room CC12 = 23.250 x 19.000 = 441.750 minus (encumbered: 3.750 x 5.500 = 20.625) = 421.125

Room CC13 = 23.250 x 19.000 = 441.750 minus (encumbered: 3.250 x 4.250 = 13.812) = 427.938

Room CC14 = 23.250 x 19.000 = 441.750 minus (encumbered: 3.750 x 5.583 = 20.936) = 420.814

Facility has 4 sinks (60) and 4 toilets & 2 urinals (90) available for the children in the Facility. LPAs reminded Sara that the children's stall bathroom doors will need to remain open to allow for children to be adequately supervised while in the bathroom. The sinks used by the preschool children have hot/cold running water. Sara understands that the hot water temperature in the bathroom sinks must be less than 120 degrees Fahrenheit at all times. Staff have one separate bathroom within the Facility. One of the staff bathrooms will be utilized by sick children if needed. Sick children will be isolated in the office area of the Facility and isolation equipment will be provided.

All Rooms have appropriate lighting, central heating/cooling, and ventilation. There no wall heater or fireplace units in the Facility. First aid supplies are stored in each Room inaccessible to children. Cleaning supplies are stored throughout the Facility in locked inaccessible to children. Sara states that there no poisons at the Facility. LPAs observed solid waste containers with tight-fitting lids in each Room,. The Facility has functioning smoke and carbon monoxide detectors. Medications will be stored in each classroom inaccessible to children.

There are 15 tables, 55 chairs, 40 cubbies, and 40 mats for the children. There is adequate equipment, supplies, and toys for children in each classroom. Drinking water will be readily available for the children indoors via reusable water bottles which the Facility will refill as needed. Transportation will not be provided by the Facility.

Sara states that the Facility does not provide any food preparation. Sara states that parents will provide all meals, including AM/PM snacks and lunch. Sara understands that any food brought from home shall be labeled with each child's name and properly stored.

Outdoor Space (Playground):

(53.333 x 19.750 = 1,053.326) + (28.583 x 99.000 = 2,829.717) = 3,883.043

TOTAL OUTDOOR SPACE = 3,661.282 sq. ft. divided by 75 = 48 children

The preschool playground to be used by the children is surrounded by appropriate fencing. The outdoor surfaces are safe for the children. There is adequate equipment, supplies, and toys for children in the preschool playground. Shade is provided by the building overhang and portable canopies. There are two play structures in the preschool playground that are safe, secure, and age appropriate. The areas around and under the play structure are cushioned with sufficient resilient materials (rubber chips). Drinking water will be readily available for the children outdoors via reusable water bottles which the Facility will refill as needed.. LPAs observed no bodies of water in the playground area of the Facility.

This Facility does provide Incidental Medical Services – IMS. The Plan of Operation and a copy of the IMS plan is on file under Facility #444414415). Sara Balla, Applicant Representative, was reminded that all adults 18 and over responsible for administration or direct supervision of staff, persons who provides care and supervision to children, and staff who have contact with children, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

LPAs reviewed with Applicant Representative the LIC 311A, Records To Be Maintained At The Facility, for child’s records, personnel records, administrative records, and documents to be posted.

A fire clearance approval for 40 preschool children was received from the City of Santa Cruz Department prior to today's inspection.

Exit interview conducted and report was reviewed with Sara Balla, Applicant Representative, and Sara was advised that a license for 40 preschool children is approved effective today, Friday January 20, 2023. LPAs advised Sara that a license will be mailed to the Facility within one week.
