On 05/27/2026 Licensing program analysts (LPAs), Jaime Gonzales and Diana De Leon were greeted and granted access to the facility by Staff, Stephanie Hernandez- Lopez. LPAs stated the reason for today's visit was to conduct a complaint investigation. Upon arrival LPAs took a census and there were four infants with one staff in the Tiny Twinkles room and three toddlers with three staff in the Rocket Ranger room. S4 was also on site during the inspection. Owner, Lily Mead took over for staff during the remainder of the investigation.

LPA Diana De Leon conducted a physical plant tour with S4 to the snack room where snacks were stored in the facility that is also shared with the church. LPA observed a fridge that is to the left hand side when facing the entrance of the room that S4 stated is used by the facility. LPA observed whole milk that expired on 05/07/2026.

LPAs conducted a file review for S1, S2 and S3. S1 has16 semester ECE units that would qualifiy S1 as a preschool teacher. S1 is currently taking the infant/toddler course and will complete the course in June 2026. S2 has 8 ECE semester units and is currently enrolled in an infant/toddler course and will complete this in the summer of 2026 S3 has no ECE units. LPA observed S1, S2 and S3 in Toddler classroom during the inspection.

Based on LPAs observations, record review evidence standard has been met, therefore the above allegations are found to be SUBSTANTIATED.

Deficiencies were cited during todays investigation. Exit interview conducted, appeal rights provided, report was reviewed with Owner, Lily Mead.
