25 | On 11/01/2024 at 10:00am, Licensing Program Analyst (LPA) Nicolette Cunningham conducted an unannounced inspection. A non-compliance conference was held with the licensee on 12/18/23 after the licensee received multiple type A citations.
During today's inspection, the licensee was providing care for four preschool age children and two infants.
The following deficiencies were cited sleep checks not documented, unsafe sleep environment, infant sleeping with toys and infant sleeping on soiled bedding (see LIC 809D).
LPA Cunningham informed licensee Sherrie Duncan that this report dated 11/1/24 documents four Type A citation(s) which shall be posted for 30 consecutive days as there is an immediate risk to the health, safety, or personal rights of children in care.
Also, LPA Cunningham informed the licensee to provide a copy of this licensing report dated 11/1/24 that documents four Type A citation(s) to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification.
Exit interview conducted and report was reviewed with the licensee. A notice of site visit was given and must remain posted for 30 days. Appeal rights were provided. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. |
