25 | On 11/8/24 at 9:15am, Licensing Program Analyst (LPA) Nicolette Cunningham conducted an unannounced inspection, and met with licensee Sherrie Duncan. Today's visit was conducted to verify the plan of correction for four Type A citations that were issued on 11/1/24 for sleep checks not documented, unsafe sleep environment, infant sleeping with toys and infant sleeping on soiled bedding.
During today's visit LPA Cunningham toured facility and observed the licensee providing care for four infants (C1, C2, C3 and C6) and three children. The licensee was operating out of ratio. LPA observed two infants (C2 & C3) sleeping in pack and plays with a bottle which is considered an unsafe sleeping environment. LPA photographed C3 sleeping with a bottle. LPA provided capacity regulations, a copy of regulation 102425 and a copy of regulation 102416.5.
The following deficiencies were cited: operating over capacity and infants sleeping in an unsafe environment (see LIC 809D):
LPA Cunningham informed licensee S. Duncan that this report dated 11/8/45 documents two Type A citations which shall be posted for 30 consecutive days as there is an immediate risk to the health, safety, or personal rights of children in care.
Also, LPA Cunningham informed the licensee S. Duncan to provide a copy of this licensing report dated 11/8/24 that documents any Type A citation(s) to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification.
Exit interview conducted and report was reviewed with the licensee. Appeal rights were provided.
