The facility pre-licensing inspection was conducted in-person on 7/28/21 at 12:00 PM by Licensing Program Analyst (LPA), Sandy Husband. The inspection is being conducted in response to the Department's receipt of an application on 7/2/21. LPA met with new applicant, Amanda Parker. The applicant has requested a preschool license for a capacity of 48. The center will operate from 7:30 AM AM to 5:30 PM, M-F The building was toured inside and outside. The preschool facility consists of 2 classrooms. The fire clearance was approved on 7/8/21 for 48 children. The preschool classrooms measured for 55 children, outdoor playground measured for 62 children. The facility is approved for the requested capacity of 48 children based on the fire clearance. Required postings (Parents Personal Rights, Emergency Disaster Plan, Earthquake Preparedness Checklist, Menu, etc.) listed on Form LIC 311A are posted. The applicant was advised that all staff are required to have a criminal background clearance on file with Community Care Licensing (CCL). The applicant is aware of the immediate $100 per day civil penalty for anyone providing care or supervision without a criminal record clearance.
The applicant completed the lead training on 7/27/21 in order to bring the Preventative Health Practices into compliance. The Director's Pediatric CPR/1st Aid expires on 10/10/22 and mandated reporter training expires on 7/27/23. Children will use the fenced outdoor play area. Sign-In/Out procedures were reviewed. The applicant/director understands that if a manual Sign-In/Out procedure is used, full legal signatures are required. Cubbies will be available for children to use. The office will be used as the isolation area for sick children. There is a separate staff restroom down the hall. There are 3 toilets, 1 urinal and 6 sinks available for children's use.

The applicant/director stated poisons will be locked in the kitchen cabinet. There is a working smoke detector, carbon monoxide detector and charged fire extinguisher rated at least 2A10BC in the facility. All Exits are marked. Items that could pose a danger to children (cleaning compounds, medication, etc) will be inaccessible. Children will nap on cots. Breakfast, lunch and a PM snack will be provided by the facility and prepared in the on site kitchen. The kitchen was inspected. Food was being properly stored and prep areas were sanitary. A sample menu is on file. The outdoor play yard measured for 62 children. There is safe toys and age appropriate equipment and sufficient shade. There is uncontaminated drinking water available for children both indoor and outdoor via individual labeled water bottles. There is wood chip cushioning under climbing structures to absorb falls. There are no bodies of water located on the property and none are to be added without prior notification and approval of the licensing agency. Unusual Incident Reporting was discussed. Children and staff records to be maintained were reviewed. Staff qualifications and ratio requirements were reviewed. Control of property is on file. This report, along with Lead Exposure Testing Flyer, COVID protocols and the self-assessment were reviewed and discussed with the applicant/director. All licensing reports are public information and must be made available upon request for at least three years. Based on the facility measurements, capacity will be granted for 48 children.

Any proposed changes to the physical plant, telephone number, or change of address shall be immediately reported to the Department



Notice of Site Visit shall be posted for 30 days from today's visit.
