25 | On December 5, 2025, Licensing Program Analyst (LPA) Kayla Adkison, arrived at the facility unannounced to conduct a Required Annual Inspection. LPA met with Social Worker Supervisor, Jake Lingle and explained the purpose of the visit. The facility is a 30-day voluntary program and clients are able to come and go from the facility unassisted.
LPA Adkison and Social Worker Supervisor toured the facility together to ensure the health and safety of clients in care. Areas toured include but are not limited to: common areas, two (2) resident bedrooms, two (2) restrooms, med room, kitchen, and outdoor courtyard. LPA observed the facility to be clean and in good repair. LPA observed all walkways and ramps to be free of clutter or obstructions.
Facility has a 2-day perishable and a 7-day non-perishable amount of food. Hot water temperature was measured within required Title 22 regulations. LPA observed all medication, sharps, and toxins to all be locked and inaccessible to residents. LPA observed multiple different activities available to clients including craft supplies, books, games, movies, and musical instruments.
LPA observed working fire extinguishers, fire detectors, and carbon monoxide detectors which are hardwired into the facility and last inspected in January 2025. The last disaster drill was conducted and documented in October 2025. The facility has been conducting drills quarterly.
In the areas toured no immediate health, safety, or personal rights violations were observed.
LPA reviewed a total of four (4) residents' files and four (4) staff files which contained all required documentation.
No deficiencies are being cited as a result of today’s inspection. Exit interview conducted and a copy of this report was forwarded to facility Administrator, Kristina Black, via email.
