On July 22, 2026, Licensing Program Analyst (LPA) Ivan Avila conducted an unannounced complaint investigation visit regarding the above allegation directed by the Department. LPA Avila met with Troy
Watkins and explained the purpose of the visit.

During the investigation process, interviews and a review of records were initiated.

LPA investigated the allegations, “Staff do not ensure the facility is properly maintained.” During the facility tour, LPA did not observe any flooding in resident areas or bathrooms. Staff reported they have not experienced any flooding or water issues and reported the facility is properly maintained.

LPA investigated the allegations, “Staff do not ensure medication is inaccessible to residents.” Staff reported that all staff adhere to their company policy regarding medications. Staff reported that medications are locked and inaccessible to residents. Staff reported that residents are given medications during meals one resident at a time and the medication cart is locked at the facility.

Based on the evidence provided, the preponderance of evidence standards was not met, therefore, the above allegations are found to be UNFOUNDED. An unfounded allegation means that the allegation was false, could not have happened and/or is without a reasonable basis.

Exit interview conducted. A copy of this report was left at the facility.

Staff reported adequate staffing and did not have issues with assisting residents with toileting needs. Staff reported they assist residents with toileting needs when residents ask staff for help. Staff reported they respond to residents toileting needs timely.

Based on interviews conducted and record review, the preponderance of evidence standards has not been met. Therefore, the above allegations are found to be UNSUBSTANTIATED. Findings that the complaint is Unsubstantiated means that, although the allegation may have happened or is valid, there is not a preponderance of the evidence to prove that the alleged violation occurred.

An exit interview was conducted, and a copy of the report was provided.

Staff reported they follow company policy of two-hour checks or as needed.
