13 | On July 22, 2026, Licensing Program Analyst (LPA) Ivan Avila conducted an unannounced complaint investigation visit regarding the above allegation directed by the Department. LPA Avila met with Troy Watkins and explained the purpose of the visit.
During the investigation process, interviews and a review of records were initiated.
LPA investigated the allegations, “Staff are not meeting residents needs.” Staff reported that resident care needs are being met. Staff reported there are multiple staff working on each shift to meet resident needs. LPA observed multiple staff working at the facility during the time of the investigation. Staff reported there have been staff in the past that did not perform their daily duties during shifts and other staff members had to perform extra duties.
Based on interviews conducted and record review, the preponderance of evidence standards has not been met. Therefore, the above allegation is found to be UNSUBSTANTIATED. Findings that the complaint is Unsubstantiated means that, although the allegation may have happened or is valid, there is not a preponderance of the evidence to prove that the alleged violation occurred.
