32 | At 3:00PM and 3:15PM, staff tried again to properly change C1’s diaper but the Diapering Log stated that C1 “Refused”. In the “Notes” part of the Diapering Log, it notes all the different ways Staff tried to coax C1 into diapering. Since the date of occurrence S1 stated C1 has been very cooperative with staff and that the new diapering procedures are helping to mediate the issues. During today’s visit the facility was toured and the diaper changing area was observed. C1’s file was reviewed, including a diapering plan that stated, site name, site phone number, child’s name, DOB, mental/physical impairments, dietary accommodations, foods to be omitted, alternatives to use.
Based on the evidence obtained, the preponderance of evidence standard has been met, therefore the above allegation is found to be substantiated. California Code of Regulations, Title 22 is cited on the attached LIC 9099D. Exit interview was conducted, notice of site visit posted, and appeal rights were provided. |
