25 | On 9/26/24 at 11:45am, an annual inspection was made to the facility by Licensing Program Analyst (LPA), Elizabeth Friese. Facility is privately funded. Operating hours are 9:00am to 1:00pm, Monday–Thursday. The facility was toured at 11:45am inside and outside and the floor and yard plan submitted by the licensee were verified. Facility operates in 2 classrooms on the grounds of the Community Presbyterian Church.
Two teachers and an aide were supervising 11 children and operating within the licensed capacity and ratio requirements. There are no pools or bodies of water on the premises. The outdoor activity space was cushioned with artificial grass, sand and bark and free of hazards.
2 children's records were reviewed at 12:05pm. 3 staff records were reviewed at 12:30pm.
The following 5 Type B deficiencies were cited, all by staff file review: 3 of 3 reviewed missing mandated reporter certificates, 3 of 3 missing TB tests, 2 of 3 missing LIC 503 (physician reports, S2 and S3), 3 of 3 missing immunizations, and 1 of 3 (S1) missing educational transcripts.
