Licensing Program Analyst (LPA) Selena Mariani was present at the facility to conduct a complaint investigation and met with Center Director (CD) Wendy Certeza. At 9:34 am, while touring the inside of the Discovery Preschool classroom, LPA observed a diaper on the bathroom floor with no child present, a box on the floor next to the hand washing station that is soap stained. The rubber floor mat by the changing table had white debris and excessive dirt and the red carpet floor mat by the door leading to the outside had white stains and excessive dirt and sensory table carpet with a lot of debris on it as well. CD stated the classrooms are vacuumed every night and there is a classroom cleaning checklist that is used each week which had not been filled out for the classroom this week.

Exit interview conducted and report was reviewed with the CD Wendy Certeza. A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. The following violation(s) of the California Code of Regulations, Title 22; Division 12, was observed during today’s visit. See LIC 809-D. Appeal Rights were provided.

Based on LPA's observation the facility did not comply with the section cited above. The diaper on the bathroom floor, rubber mats and cartet floor mats with exessive dirt and soap stained cardboard box, which poses a potential health, safety, or personal rights risk to persons in care.
