13 | Licensing Program Analyst (LPA) Dianne Allee made an unannounced complaint investigation visit today and met with Licensee Nina Simmons (LS) for the purpose of delivering findings for the above allegations. LPAs Dianne Allee and Leticia Rosales-Meza previously met with Licensee Nina Simmons on 03/20/2026 to open the complaint.
During the course of the investigation, LPAs Allee and Rosales-Meza conducted interviews and received documents pertaining to the investigation. From 03/20/2026 through 06/09/2026, interviews were conducted with Licensee (LS), staff (S1), three parents (P1, P2 & P4) and an additional adult interview was attempted.
Licensee (LS) denied the allegations. LS stated she recently had an incident with a family regarding her sick policy, which lead into the parent yelling from her car at LS. Staff interview (S1), stated that they only interact with children occasionally and are never left alone with them. S1 further stated having positive interactions with the children.
Parent interviews (P2 & P4) stated that they have never observed or heard of LS or S1 yelling at children. P2 further stated they have never witnessed disagreements or inappropriate interactions between LS and any parent during drop off or pick up.. While P4 stated there was an incident reported by their child in which a parent entered the facility yelling at LS, which scared their child. In addition, P4 stated they arrived to the facility to pick up their child observing a parent leaving the facility upset further stating that was the only incident they have observed between LS and a parent.
Based on the information gathered during this investigation, although the allegation may have happened or is valid, there is not a preponderance of evidence to prove that the allegation occurred and therefore is determined to be unsubstantiated. There were no Title 22 deficiencies cited. This report was reviewed and discussed with Licensee, Nina Simmons. Appeal rights were provided.
Notice of Site Visit shall be posted for 30 days from today's visit. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
