32 | Center Director stated even though both parents were present an incident report was created. Center Director stated C1 was never injured, it was always playful behavior with C1's friend, Child 2 (C2). Both C1 and C2 were always physically active, either bumping shoulders, grabbing hands and leading each other to play areas. Center Director stated this is the only time an incident report was necessary. This incident didn't meet the guidelines to report to Licensing. S1 corroborated the center directors’ statements. S1 stated when an incident occurs they write up what they witnessed and input it into the "Procare" application.
Licensing Program Analyst (LPA), Leticia Rosales attempted interviews with nine parents, Parents 1 - 9 (P1-P9) between 08/08/25 to 10/28/25. Interviews were conducted with two parents (P10 and P11) on 10/31/25. Parents revealed they are usually informed in person or through the "Procare" app when their children have been injured, but believed their child was injured due to their own accord not due to lack of supervision. Parents revealed staff members are very communicative and they are always supervising the children. Interviews were conducted with three children. Children interviews revealed when they get hurt, or someone gets hurt they get a bandage and sometimes they receive an ice pack from the teachers. Children interviews corroborated who the staff are in their classroom, and how teachers tend to when an injury occurs.
Based on interviews conducted, although the allegation may have happened or is valid, there is not a preponderance of the evidence to prove that the alleged violation occurred, therefore the allegation is Unsubstantiated. There was no Title 22 deficiency cited based on the above findings.
Exit interview conducted and report was reviewed and discussed with Facility Representative, Tawyna Archer. Appeal Rights were provided.
Notice of Site Visit shall be posted for 30 days from today's visit. Failure to comply with posting requirements shall result in a $100 civil penalty.
