32 | Interviews were conducted with five guardians ( G1-G5) and one child ( C2) between 04/06/2023- 06/01/2023. Four guardians reported when their children have gotten hurt in the day care S1 has told them about, and provided an ice pack when needed. One guardian reported their child came home with a large bump on their head and were told by S1 that the child fell, but on a different occasion child arrived home with scars on their feet and the day care providers were not able to explain how it happened. C2 reported when a child gets injured in the day care S1 and S2 Kiss the site of injury. One guardian reported when their child was injured, S1 was the only provider present but didn’t report how many other children were present. Three guardians (G1,G2,G4) reported when they drop of their children in the morning before 08:30AM, the only provider present is S1. Interviews corroborated while G1, G2, G4 are dropping off their children S1 is walking back and forth from the kitchen to the day care room with waters, or other items for the day care room. Interviews revealed this has only happened a few times. Guardians reported during that time, S1 is only out of the room for no longer than two minutes and guardians will wait in the day care room before leaving so S1 can see the guardian drop off child and leave. G1, G2, G4 all reported S2 does not arrive to the facility until 08:30 AM, and while they are dropping off their children there are about 3-4 children in the day care room watching TV. G3 reported the only time children have been left unattended is when they are going to the bathroom which is down the hall from the day care room. Interviews reveal three guardians believe there is adequate supervision in the day care. One guardian reported they don’t have much interaction with S2, so their experience has been based solely on interaction with S1. LPA attempted to interview two more daycare children over the phone, and their guardians denied the request.
LPA arrived the facility on 06/08/2023 to make observations and conduct additional interviews. LPA arrived to the facility at 08:14AM and observed S1 in the day care room with eight children. The children’s water bottles were placed on the table and children were co-mingling. S1 stayed in the room and was interacting with guardians, as guardians were arriving to drop off children. At 08:35 AM S2 arrived to the facility, as soon as S2 arrived they went to the kitchen and began to heat up the children’s breakfast for the day. While S2 was in the kitchen S1 had children sit in their designated area based on age and helped S2 serve children waffles.
Based on interviews conducted and records reviewed, although the allegation may have happened or is valid, there is not a preponderance of evidence to prove that the alleged violations occurred, therefore the allegations are Unsubstantiated.

There were no Title 22 deficiencies cited during today’s inspection. |
