32 | Six staff (S1-S6) were interviewed on 10/7/25 and 12/17/25. S6 stated that children are always supervised and there has never been a time where they have been alone in the classroom while a child needed a diaper change. S1 stated that they would be able to get support if they were going to be out of ratio. S4 stated that if they needed help in their classroom, they could call Vicki to support them. Two staff (S2 and S5) stated that one teacher is the only teacher in their class in the afternoon. S2 and S5 stated that one teacher sometimes does not have coverage in the classroom and will change diapers while children are unsupervised. S1 stated that after a child is injured, the child is assisted and then a report is written up and parents are notified. S3 stated they wrote up incident reports when children are injured. One staff stated they were told not to disclose an injury to a parent. S4 and S5 stated they will send a message to a child's parent after a child is injured or hurt.
Three children (C1-C3) were interviewed on 12/17/25. One child (C1) stated they only have one teacher in their class and that teacher changes diapers in the bathroom and in the classroom. C1 stated they are with their friends while their teacher changes diapers.
Six parents (P1-P6) were interviewed on 12/30/25 and 1/7/26. P1, P2, P4, P5 and P6 stated that they have not witnessed children being left unsupervised. P3 stated that they have witnessed children having their diaper changed while other children in the class were unsupervised. No parent interview stated that they were not made aware verbally or by incident report when their children have been injured while in care. P6 stated that their child has never returned from the facility with unexplained injuries.
Based on the evidence reviewed the findings are unsubstantiated. Although the allegation may have happened or is valid, there is not a preponderance of the evidence to prove that the alleged violations occurred. Appeal rights were provided and exit interview conducted.
The Notice of Site Visit must be posted for 30 days. |
