25 | Licensing Program Analyst ( LPA) Elpidia Hernandez Torres arrived to the center to conduct a case management visit and discuss Unusual incident/ Injury reports that were received in February and March 2025. The center reported there were incidents occurring repeatedly with the same children, resulting in injuries to themselves or others. LPA arrived to interview Center Director and staff to further identify if the incidents occurring were preventable or accidents.
After interviewing Center Director and Staff, the incidents from February were already being addressed with guardians, children have begun to receive additional support services where needed. The center also confirmed the injuries from February were accidents.
There were a few incidents that were reported in March 2025, which LPA identified didn't warrant an Unusual incident/ injury report and reviewed those cases with center director. LPA received further information regarding incidents that occurred with children bumping into bathroom wall and book case desk, which were accidents that resulted in small head bumps/ bruises. After staff explanation, LPA reminded staff to be cautious of the book case desk, which can result in an injury if not put away after use.
There were no deficiencies issued during todays visit. LPA provided notice of site visit which is required to be posted for 30 days. |
