25 | During the course of an investigation, Licensing Program Analyst (LPA) Elpidia Hernandez Torres, was present at the facility on 07/11/2024 and observed wasps flying around and entering tubes of the permanent outdoor canopy, which is used to provide shade for children. Upon asking staff about the wasps, staff reported it has been an issue since May 2024. Staff reported they had brought it to the attention of the center director in May 2024, and the assistant center director towards the end of June 2024. As of July 12th 2024, assistant center director placed traps on the canopy, but that did not resolve the issue.
In addition, on or around June 26th-June 28th, a child sustained an injury to a finger which resulted in first aid which was caused by metal mesh fence that was in dis-repair, not attached to the metal rod fencing allowing for kids to put their hands through the fence and scraping their skin on the exposed metal mesh fence. Staff reported they had brought the exposed metal mesh fence to the attention of the center director in the beginning of June, and offered to fix the fence but center director declined until owners could be contacted. LPA issued type B deficiency on the attached 809-D.
LPA toured the outdoor play space, and observed the tubes of the canopy were taped to prevent wasps from entering the tubes and creating nests inside. The wire metal mesh was zip tied to the metal rod fencing to prevent children from putting their hands/ arms through. Exit interview was conducted with Center director, she was provide a copy of this report and appeal rights. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
