A complaint investigation visit was made today by Licensing Program Analyst (LPA), Jessica Gaumann who met with Director, Lysandrea Jackson for the purpose of delivering complaint investigation findings for the above allegation. LPA met with Director on 05/05/26 to initiate the investigation by discussing the allegation, conducting interviews, making observations, and requesting documents.

At today’s inspection, the facility was toured inside. During today’s visit, LPA observed 17 sleeping children, of which 3 were awake, being supervised by two unqualified staff with no units.

In addition, on 05/05/26 LPA observed 19 children being supervised by one qualified teacher and one unqualified aide.

During the course of the investigation, LPA conducted interviews with the Director (D1), 3 staff (S1 – S3) and reviewed & received records. D1 stated that due to staffing issues, the facility has operated the preschool classroom out of ratio on multiple dates. S1 stated they were left alone with more that 12 day care children at one time on multiple dates. S2 stated that she was an Aide. S3 stated she does not have any Early Childhood Education units (ECE). Record review revealed there were no ECE units on file for S2 or S3 at the facility.

Based on the information gathered during this investigation, the preponderance of the evidence standard has been met. Therefore, the allegation is determined to be substantiated. California Code of Regulations, Title 22, is being cited on the attached LIC 9099-D. Appeal rights were provided. An exit interview was conducted, and this report was read and discussed with the facility’s Director, Lysandrea Jackson. A notice of site visit was given and must remain posted for 30 days.
