25 | Licensing Program Analyst (LPA) Walters conducted an unannounced Annual Required – 1 yr. Infection Control inspection to this facility and was greeted by Staff, Lexie Wilridge. Administrator, Dana Thomas arrived later. LPA conducted a Risk Assessment with staff. At the time of inspection there was 1 staff providing care and supervision for 4 clients.
Upon entering the facility, LPA observed a sign stating that the facility is no longer allowing visitors. LPA and Administrator discussed the PIN lifting the no visitor policy, and the importance of visitors signing in. Staff screened LPA Walters for COVID-19 symptoms and checked LPA's temperature and signed LPA into the facility.
At approximately 9:00 AM LPA and S1 toured the facility. LPA observed signs promoting social distancing and hand washing. Facility was a comfortable temperature and exits were free from obstructions. Facility has a 100% vaccination rate of staff and residents. Vaccination records are stored. Administrator is in the process of scheduling booster shots for clients. Clients do not typically wear a mask while in the facility, but they do wear masks when in the community. 1 client attends day program two times a week, facility also provides activities for clients when they are at home. Clients have access to alternative communications, phone calls and video calls with their families. Facility has submitted their Covid Mitigation Plan and approved on 3/24/21. Facility has more than a 30 day supply of Personal Protective Equipment (PPE) and incontinence products including: masks, face shields, gowns and hand sanitizer. PPE supplies are located in an accessible place for all staff.
