At approximately 2:45PM, Licensing Program Analysts (LPAs) Felias and Sarangi arrived unannounced to conduct a Case-Management Plan of Correction (POC) visit and met with Administrator, Stephanie Henry.

LPAs discussed the deficiency cited during a visit conducted on 6/28/2022. Administrator stated documentation of training could not be found but that staff recently had training completed. Administrator provided LPAs with Documentation of Training dated and completed on 7/31/2022.

Deficiency cleared during visit.

No deficiencies cited during this inspection.

Exit interview conducted. Copy of report discussed and provided to Administrator. Signature on form confirms receipt of documents.
