Staff also indicated they have heard reports of other pests being found, however, when a pest is seen or reported it will be elevated to management to have the pest control come out. Further interviews with staff indicated they will observe pests in the fields that surround the facility, and they will make sure to close doors, however, some residents have direct access to these fields from their rooms and pests may enter through there. Review of pest sighting log indicated that pests have been observed in the facility, however, they are reported by staff and residents, and further steps are taken. Review of third part pest control service report indicated that pest control will come out when called to exterminate pests and install preventative measures. Further review of report indicated that pest traps outside of the facility were consumed.
Resident's needs were not being met – RP alleges that resident (R1) did not have their needs met resulting in medication errors, lack of eating, and lack of bathing. RP also alleged possession of a video showing R1 not being fed for multiple days. LPA was not able to acquire video after numerous attempts. Interviews with management indicated that R1 would often refuse to leave their room for meals and would have them brought to their room. R1 would also refuse showers and required staff prompting, facility policy is to prompt three (3) when a resident refuses an Activity of Daily Living (ADL). Review of Physician Report indicated that R1 is ambulatory and able to bathe themselves. Review of care plan indicated R1 requires stand-by assistance only with bathing and will refuse frequently requiring staff prompting. Review of Resident monthly completed tasks indicated that R1 received frequent bathing and food services. Review of Medication Administration Record (MAR) indicated that R1 was receiving medication as prescribed.
Although the allegations may have happened or are valid, there is not a preponderance of evidence to prove the alleged violations did or did not occur, therefore the allegations are unsubstantiated.
No deficiencies cited. Exit interview conducted with Regional Director of Operation, whose signature on form confirms receipt.
