13 | Licensing Program Analyst Leibert arrived unannounced for the purpose of delivering findings on this complaint. During the course of this investigation, site visits were made to the facility, statements were taken and documents obtained and reviewed. The following deteerminations are made: Complainant alleges that the staff have neglected to provide a shower for R1 on a regular basis in compliance with R1's care plan; Monthly Care Plan logs for the month of August, 2023, lack documentation of showers provided for R1 from 8/16 thru 8/31. Based upon the documents obtained and reviewed, the preponderance of evidence standard has been met. Therefore, the allegation is SUBSTANTIATED.
The following deficiencies were observed (see LIC 9099D) and cited from the California Code of Regulations, Title 22, Division 6 of California Regulation. Failure to correct the deficiency and/or repeat deficiencies within a 12 month period may result in civil penalties. Exit interview conducted and appeal of rights provided.
