During a subsequent investigation inspection visit on 08/22/25 LPA toured the facility, obtained documents and conducted interviews with four children, C2-C4 and C6 and one staff, S2.
During the interview on 06/13/25, D1 stated that all children nap in the same classroom, classroom #1 where one staff, S1 was present and S2 was on a lunch break. D1 stated that on 06/05/25 she recalled having multiple children need to leave the classroom due to disruptive behavior at nap time and come to her office for about three minutes per visit. D1 shared that she recalled that child C1 was asked to sit on the floor in the “Director’s Office” the room that has the copier in it, while child C2 was asked to sit on the floor of the “Main Office” where D1’s desk is. D1 stated that she sat at her desk and was able to see both C1 and C2. D1 recalled that a third child, C3 had also had a 3 minute visit to her office during nap time, sitting in the “Main Office”. D1 stated that C1 and C2 were brought to the office twice during nap on 06/05/25 and C3 just once. D1 confirmed that the lights in the office were turned off, just as they are in the classroom during nap time to keep the calming effect. D1 stated that it was not completely dark due to the light coming from her computer screen, the Christmas style lights and the light coming from under the door in the “Director’s Office”, a door that leads to the Fireside Room and had a lot of natural light at that time of the day. D1 reported that none of the children cried in the office spaces or said that it was too dark. D1 stated that she did have children walking back and forth with her to the classroom, but no children were left unsupervised in the office spaces.
Interviews with staff provided conflicting information. One staff interviewed stated that she thought it was possible that a child was in the office unsupervised while D1 escorted another child back to the classroom or helped a child in the bathroom, but also recalled only two of the same children, C1 and C2, as having gone to the office during nap time on 06/05/25, then was reported by D1, and named child C4 as the third child removed from the classroom for a visit to the office on 06/05/25, not C3, as stated by D1. The other staff interviewed stated that they have observed, on more than one occasion, children sitting on the floor in the office, while D1 was sitting at her desk, stating that they have never seen a child unsupervised in the office. S1 reported that none of the children said anything about their time in the office, and none of them appeared upset when they returned to the nap room.
