25 | Licensing Program Analyst (LPA) Amy Strother made an unannounced case management visit to the facility after receiving an Unusual Incident Report (UIR) on 03/24/25, regarding a possible violation of children’s personal rights of child, C1. LPA met with Facility Representative, Director, Melinda Keena (D1).
The UIR dated 03/24/25 stated that the alleged incident took place on 03/05/25. Included with the UIR was a typed statement from staff, S1. After review of the UIR, LPA determined that additional and clarifying information was needed. During today’s visit, LPA toured the facility, 12 students were being supervised by 3 teachers, operating within the licensed capacity and ratio requirements. LPA interviewed D1, Staff 2 & Staff 3 (S2 & S3) and attempted to interview one child, C1. LPA was not able to qualify C1 for interview. During today's visit, D1 stated the alleged incident occurred on the playground between 10:30am and 11:30am, stating that during that time 15 children were being supervised by two teachers and an aide, S1-S3. Staff interviewed did not make any statements that supported a violation of children's personal rights. LPA requested and received a current roster of children in care, LIC9040 and current Personnel Report, LIC500.
Based on interviews conducted and records reviewed, there was not a preponderance of evidence to prove that an alleged violation of personal right occurred.
There were no Title 22 deficiencies cited during today's inspection.
This report was reviewed and discussed with Director, Melinda Keena. Appeal Rights were provided.
Notice of Site Visit shall be posted for 30 days from today's visit. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
