13 | A subsequent complaint investigation inspection was conducted by Licensing Program Analyst Jennifer Velasco (LPA), who met with Staff Sarah Welton (S5). It has been alleged the license is not posted; specifically, staff has not posted the facility license for viewing by parents. It has been further alleged children have sustained injuries in care; specifically, multiple children have sustained minor injuries on the playground due to inadequate staffing. S5 stated the facility license has always been posted in the central hallway and that parents have regular access to that central hallway. S5 also stated while some children have sustained minor injuries while in care, those injuries have been minor and sustained in the normal course of play. During the investigation, LPA toured the facility, conducted witness interviews, and requested pertinent documents and materials. Witness statements, documentation, and observation failed to corroborate the allegations that the facility failed to posted the license publicly and that children sustained injuries outside the norm while in care. Based on interviews and facility documents, the preponderance of evidence standard has not been met; therefore, the above allegations are found to be UNSUBSTANTIATED. This report was reviewed and discussed with S5. Appeal Rights were provided, and exit interview was conducted. All licensing reports are public information and must be made available upon request for at least three years. S5 was provided with a Notice of Site Visit (NOS) to be posted in the facility for 30 days. |
