13 | Licensing Program Analyst (LPA) Glenn Ouye conducted an unannounced visit today and met with licensee Nade Ali to complete the complaint investigation and deliver findings for the above allegations. LPA Phouthavong previously met with the licensee on February 26, 2025 to initiate the complaint investigation. During today's visit on 3/11/25, LPA is substantiating the allegation that staff left children unattended. On or about 12:40pm on February 26, 2025, LPA Phouthavong asked S1 for a copy of the facility children's roster. S1 and S2 entered the facility kitchen to look for the roster. LPA Phouthavong then observed child C1 enter the green painted 3 year old classroom to the left of the bathroom without the staff knowledge. LPA followed C1 into the 3 year old classroom and observed C1 on a table reaching for a toy. LPA was asking the child to come down from the table and return to the main classroom when the child fell into LPA Phouthavong arms. LPA helped the child back to the ground and asked for S1 and S2 to enter the 3 year old classroom so staff would be aware that the child was left without supervision. Based on the observation of LPA Phouthavong at 12:40pm on 2/26/25 that child C1 was without supervision in the 3 year old classroom, there is a preponderance of evidence to support that staff left children unattended and the allegation is substantiated.
