25 | On 02/14/24 Center Director, Catherine Schlager (D1) informed LPA Strother by email that she would like to add an additional outdoor space, the “Apple Play Garden” to the facilities license. On 02/15/24 LPA attempted to contact D1 leaving a voicemail. On 02/26/24 D1 contacted LPA Strother by phone stating that she will submit an updated Facility Sketch (Yard), form LIC999 to include the new play garden. D1 stated during the call that the space is currently used for exempt from licensure care, by a group of younger children that gather with their parents for short session from 9:15am to 11:30am. The licensed facility would like to use the space on a schedule separate from the exempt care, for a preschool group during drop off and pick up from 8:30am – 9:15am and again from 11:30am- 12:30pm. During today’s visit, LPA toured the play garden. The area was free of hazards during the time of the inspection. D1 stated that she will maintain the space in compliance with the regulations and health & safety codes of the facility license. LPA was provided with an updated sketch to include the Apple Play Garden, along with a written schedule of times the space will be used by each group. D1 stated that she understands that any changes in the scheduled use should be reported to Licensing and placed in the facility file. The space “Apple Play Garden” has been approved for use.
A notice of site visit was given to facility representative and must remain posted on, or immediately adjacent to, the interior side of the main door for 30 days.
