On October 2, 2025, Licensing Program Analyst (LPA) Y. Yang conducted a follow-up investigation visit to present findings from a previously initiated investigation. The initial visit occurred on July 17, 2025, following allegations that staff handled children roughly, failed to provide a safe environment, and did not treat children with dignity and respect. Specifically, it was alleged that on a certain day, staff member S2 spoke to children in a harsh and inappropriate tone, forcefully placed children in their seats, roughly grabbed a cup from a child, causing the child to cry, and forcefully removed food from a child's mouth. These actions were reportedly witnessed by the licensee, Gwendolen Foursha, who did not intervene or correct S2's behavior.

During today’s visit, the LPA met with Licensee Gwendolen Foursha (Staff S1) to review the investigation’s findings. During the initial site visit on July 17, 2025, the licensee was interviewed and denied all allegations. She confirmed she was present at the facility at the time of the alleged incidents but insisted she did not believe that staff S2 conducted themselves inappropriately or handled children roughly. The licensee stated that, if she ever observed a staff member behaving improperly, she would immediately intervene and give guidance on how to handle situations appropriately. The licensee also asserted that all children under her care receive safe, healthful, and comfortable accommodations and equipment, are treated with dignity in all interactions, and that corporal punishment or other cruel or unusual disciplinary actions are never permitted at the facility. During the investigation, the LPA interviewed Staff S2 about the allegations. Staff S2 denied that she ever handled children roughly or spoke to them inappropriately. Staff S2 acknowledged helping children during mealtime on the day of the alleged incidents and playfully taking items from children and interacting with them but insisted that she did not do anything improper or rough. She explained that the facility environment during lunch time and transitions between activities is often loud and chaotic, which sometimes requires her to raise her voice so the children can hear her. Staff S2 stated that that some of her actions may have been taken out of context. Staff S2 also said that there are occasions when she must use a stern tone to manage behavior, but she firmly denied ever speaking to the children in a threatening or inappropriate manner or roughly handling any child.

Throughout the course of the investigation, the LPA conducted observations and interviewed children, staff S1-S3, and other individuals familiar with the facility. The information obtained through these interviews did not yield any corroborating evidence to support the allegations. Furthermore, during the LPA’s visits to the facility on July 17 and the date of this report, no observations were made indicating any violations of children’s personal rights. The children in care appeared to be treated with dignity in their interactions with staff and others and were provided with safe, healthful, and comfortable accommodations, furnishings, and equipment.

Based on available information, although the allegations may have happened or are valid, there is not a preponderance of evidence to prove the alleged violations did or did not occur, therefore, the allegations are determined to be unsubstantiated at this time. There were no Title 22 deficiencies cited. This report was reviewed and discussed with the facility’s licensee, Gwendolen Foursha. Appeal rights were provided. Notice of Site Visit shall be posted for 30 days from today's visit.
