25 | An unannounced case management visit was conducted by Licensing Program Analyst (LPA) Y. Yang in response to a self-reported unusual incident involving Child C1 that occurred at the facility on June 16, 2026. During the visit, the LPA met with Site Supervisor Kristie Lueza to discuss the incident.
According to the Site Supervisor, the incident occurred at approximately 10:00 a.m. during outdoor play. Child C1 was running on the playground under the supervision of substitute teacher S1. While holding C1’s hand, S1 noticed that the other children were sitting down for circle time and encouraged C1 to join the group. Staff reported that C1 then dropped backward to the ground and began crying softly. The Site Supervisor stated that S1 immediately crouched down and assisted C1 as the child lowered themself to the ground to help ensure a gentle and safe fall. Teacher S2, who witnessed the incident, approached and checked on C1 before picking the child up and carrying them to circle time.
Immediately after the incident, S1 and S2 notified Site Supervisor Lueza. The Site Supervisor assessed C1 and checked the child’s range of motion. Although C1 did not appear to be in pain, the Site Supervisor observed that the child winced when their arm was moved. As a precaution, the Site Supervisor attempted to contact C1’s authorized representative several times but was unsuccessful. She then contacted the child’s emergency contact and informed her of the incident. Shortly thereafter, the authorized representative returned the call and was advised of the circumstances and staff observations. The authorized representative stated that she would meet the child’s other emergency contact at the emergency room so C1 could be evaluated by medical personnel. The Site Supervisor later learned from the authorized representative that C1 had been diagnosed with nursemaid’s elbow.
The Site Supervisor reported the incident to Community Care Licensing as required by regulation and notified the child’s authorized representative. During today’s visit, the LPA discussed the incident with the Site Supervisor, reviewed the facility’s supervision and injury-response procedures, and toured the facility. At the time of the visit, staff were observed actively supervising children and operating within required ratios.
Based on information obtained during the case management visit, staff responded appropriately by immediately addressing the incident, notifying the Site Supervisor, assessing the child’s condition, and facilitating medical evaluation through prompt communication with the family. The LPA provided technical assistance and discussed best practices with the Site Supervisor during the visit. This report was read and reviewed with Site Supervisor Kristie Lueza. A Notice of Site Visit shall be posted for 30 days.
