25 | An unannounced case management inspection visit was made to the facility by Licensing Program Analyst (LPA) Y.Yang to follow-up on a self-reported, unusual incident report (UIR) received by the department on 07/30/2021. During today’s inspection visit, LPA met with center director Karen Arena.
Per the incident report, the center's associate director, Sarah Kalua reported that on 07/29/2021 at 10:45am, child C1 was running on the preschool playground with another child when child C1 tripped and fell and landed on the playground's rubber flooring. The associate director reported that C1's teacher, staff S1 assisted the child and notified the center director. Per the UIR, the child was examined by S1 and an ice pack was provided and the child's guardian was notified. The associate director reported that C1 was picked up by their guardian approximately 30 minutes after their guardian was notified. The associate director reported that C1 was brought to an urgent care clinic by their guardian later that day and it was discovered that the child sustained a right clavicle fracture.
During today's visit, the LPA interviewed S1 who stated that she was standing near C1 during the incident and was supervising a total of 12 children on the playground at the time. S1 stated that the child was not pushed or struck by another child. During today's visit, staff were observed to be providing appropriate care and supervision and operating within ratio. LPA toured the outdoor play area and did not observe any safety or trip hazards that could have caused this injury. This incident was reported to Community Care Licensing as required. This report was read and reviewed with the center director. There were no Title 22 deficiencies cited during today's inspection. Notice of Site Visit shall be posted for 30 days from today's visit. |
