Licensing Program Manager (LPM) Mindy Mohr and Licensing Program Analyst (LPA) Jen Patel made an unannounced complaint investigation visit today, and met with Director, Eden Stein (D1), for the purpose of delivering findings of the above allegations. LPA/LPM previously met with D1 on 08/11/2025 to open the complaint. During the course of the investigation, LPA and LPM conducted interviews, received documents and made observations. From 08/11/2025 through 09/12/2025, interviews were conducted with Licensee (L1), Director (D1), one staff (S1) and five adults (A1-A5). Additional adult interviews were attempted.

Licensee denied the allegations, stating children act out fast in front of staff, which can be hard to stop at times and parents are always notified of their child’s injuries via the Brightwheel application. L1 further stated children arrive at school with bruises and will get bruises and cuts while at school. If a child is injured while at school a staff member will notify the parent via Brightwheel and will follow up with the parent at pick-up.

Staff (D1 & S1) interviews revealed the facility always maintains a ratio of one teacher to six children. D1 specifically stated the facility will close classrooms if there are not enough staff present. D1 further stated children get hurt at the facility daily. When a child gets injured while at school, the child is supported by staff and then re-enters into play. Staff then send the parents a message via the Brightwheel application. D1 stated there is not a way to confirm a message has been read by the parent, but staff will speak to the parent at pick up reading the child’s injury. S1 corroborated D1’s statement stating the facility maintains a ratio of one teacher to six children, incident reports are sent to parents via the Brightwheel application and staff speak to parents at pick-up regarding their children’s injuries. In addition, S1 stated they have had children at the school who bite other children as well as children who had issues with fighting and bullying, but staff were always there when that occurred.

Adult interviews (A1, A3 & A5) all stated their children have gotten injuries while at the facility, and were notified of the injuries via the Brightwheel application. A1 further stated they were not always notified in person, while A3 stated they were always notified when picking up their child of the injury. A2 stated they observed teachers handle situations right away when a child hits or bites another child, and that L1 worked with those families individually when incidents like that occurred.

Based on the information gathered during this investigation, although the allegation may have happened or is valid, there is not a preponderance of evidence to prove that the allegation occurred and therefore is determined to be unsubstantiated. There were no Title 22 deficiencies cited. This report was reviewed and discussed with the facility’s Director, Eden Stein. Appeal rights were provided. Notice of Site Visit shall be posted for 30 days from today's visit. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
